1/ We're going to need to look after each other really well in the weeks and months ahead.
This was highlighted to me recently when I was taking out my rubbish (trash).
I had a little stress reaction to getting a new rubbish bag. It really took me by surprise.
2/ I initially dismissed my feelings as bizarre and ridiculous.
I realised immediately that it was the tearing of one rubbish bag from the next that reminded me of tearing plastic PPE gowns off a roll.
I didn't understand why this association stressed me. So it isn't valid is it?
3/ Next time I was at work we were having a bad plastic gown roll day. You might know what I mean. The kind that rip and don't separate easily. It might take a couple of goes to get a gown and even then it can take a while.
4/ That made me think that I'm just an impatient person. It's not like I'm not being paid to test plastic gowns badly.
So now I'm just thinking that the bin bag reaction shows that I'm grumpy and intolerant.
5/ Then a few patients into my shift, I was seeing one child when a nurse poked her head in and said, "They need you in Resus. There's a sick baby and they're struggling with access." I removed my PPE, cleaned my hands and went to don new PPE.
The plastic gown roll tore again...
6/ So now I know that my stress is valid and based in something real. For that bin liner thing.
I also know that I had a stress response that I initially dismissed, then berated myself for and blamed on character flaws before coming to understand it.
7/ We're going through some crazy times right now at home and at work.
A lot of psychology will be going on in you, your colleagues and your loved ones.
Look after yourself and each other.
8/ One way to do that is to see someone who seems stressed or angry and ask them if they are ok.
There's a chance that they aren't but they don't understand why.
There's also a good chance that because they don't understand it, they don't think it's ok to feel that way.

1/ Some reflections on remote and online classroom style medical education vs face to face (F2F)

#FOAMed #MedicalEducation

A thread...
2/ The growth in remote MedEd has occurred through necessity. Remote is different but not better than F2F. It has some clear advantages but also significant disadvantages. Where possible we need to mitigate these problems.
3/ Delivery is not learning. Remote MedEd is high risk for providing something which is not received in any meaningful way. His is especially true for education which is not the agenda of the learners.

1. People often ask at the moment, "What's the exit strategy for the Thursday clap?"

Is it going to continue indefinitely?
If it stops, who decides that?

Well I have a proposal from the NHS worker's point of view.
2. How about we all agree that we stop the Thursday clap when the government meet my three key tests.
3. The first test is that NHS worker's all have their pay brought back in line with where it was 10 years ago, before it was effectively reduced year on year by below inflation pay rises.

1 With the impending #COVID19 pandemic peak in the UK, adult services are going to be hit disproportionately hard. Since paediatricians may be needed to help by looking after older patients, here's a guide to help children's specialists when the time comes to care for an adult.
2 Don't introduce yourself as a paediatrician. Most people think that's a foot doctor. Even if they know what a paediatrician is, confidence will not inspired.
3 Don't use bubbles, stickers or play specialists. These are not evidence based practice in adults.

1 A quick tweetorial on childhood immunology that might help to explain why children are less likely to be seriously unwell with #COVID19
Also a warning about neonatal Covid 19 based on what we know about novel infections in this age group.

2 There are essentially three main stages to human immune system development. The end result is a complex and highly intelligent response to pathogens. This response is most like the employment of an intelligence service to protect the body from infection.
3 The adult immune system can produce that clever and targeted response because it has been educated by thousands of infections that it has previously encountered. It is possible to further educate that immune system with vaccines.
