It was not designed to study the transmissibility of the virus post-vaccination.
Allow me to share an analogy related to the issue
Previously our pilgrims were mandated to take the Meningococcal Polysaccharide Vaccine (MPV) prior to their Hajj @drzul_albakri @imamalaysia 2/10
The MPV protected the pilgrim from Invasive Meningococcal Disease (IMD) but they are still able to transmit the disease to others around them since the MPV did not clear the bacteria from their upper airways @drzul_albakri @imamalaysia @FIMAweb 3/10
When the first Meningococcal Conjugated Vaccine (MCV) was available in Malaysia, I presented to select members of the Majlis Fatwa Kebangsaan (MFK) on the superiority of the MCV since it prevented both the vacinee and others around him from IMD @drzul_albakri @imamalaysia 4/10
The MCV eradicated the bacteria from the upper airway and conferred indirect protection to persons around the vacinee.
I hope this will now convince the LUTH and all pilgrims to take the MCV instead of the old MPV @drzul_albakri @imamalaysia @FIMAweb 5/10
Back to COVID, thus far only researchers at Oxford, using the viral vector #AstraZeneca vaccines have hinted that testing showed the vaccinated group in UK had fewer asymptomatic infections, which means they’d be less likely to spread the disease @drzul_albakri @imamalaysia 6/10
I am hopeful that mRNA #CovidVaccines will have some effect on transmission based on 3 plausible mechanisms:
1. If particles breathed out by asymptomatic/pre-sym cases during talking & breathing transmits the virus, then a vaccine that reduces symptoms wud also reduce spread 7/10
2. The mRNA #CovidVaccines by reducing a person’s viral loads in the upper airway also cuts the amount they can transmit and spread @drzul_albakri @imamalaysia @FIMAweb 8/10
3. Better still, if the #CovidVaccines act like the conjugated vaccines (MCV and PCV)), by conferring mucosal immunity, they sterilize the upper airway and prevents its emission into the air space and therby reduce the spread of the coronavirus 9/10
These I anticipate are the secondary objectives of their mRNA vaccine studies.
Hence, there is no data presently to address this issue of #COVID19 transmission by the vaccinees 10/10

To an IT-idiot like me-MySJ is like a failed App
Millions spent to promote penetration
Up to 60% as alleged by MOH
RM50 given per user
BUT only 4% as a measure of effectiveness?
Imagine if Grab spent millions to develop the App and promoted the app through RM50 voucher to all Malaysians
Only to find a mere 4% of its users using it to conduct business
It will be a FAILED APP.
If I may ask, how much taxpayer’s money was spent on this 4% MySJ?
How many were compounded RM1K for not checking-in MySJ?
Only to b told MOH didn’t use the check-in data to contact trace!
Why punish the rakyat when MySJ never even considered the info as crucial?
Why mandate us to scan it like idiots if the data points aren’t utilized optimally?

IMHO this is a culmination of old school public health tools
Think out of the box and re-strategize
Few ideas to consider:
1. Use Rapid RTK-Ag not Slow PCR testing-no backlogs
2. Universal testing not targeted testing-esp #Sabah
3. Ramp up RTK-Ag in #Sabah- 100K not enough
4. Cap the cost of RTK-Ag testing at private labs/hospitals so #NowEveryoneCanTest
5. Frequent HCW testing to protect them & patients in healthcare settings
6. More resources/HCW deployed to #Sabah. Pre-COVID #Sabah was underfunded/understaffed/underpaid/overworked
7. Make sure HCW/Volunteers feed/rest/sleep well so as to mitigate burnt-out & low morale. Pay elaun stat!
8. Set up more low risk Isolation Centres to nurse pre/asymptomatic/mild COVID cases-esp #Sabah
9. Create more COVID beds in MOH. Decant non-COVID cases to private hospitals

Nuances here more political than scientific.
IMHO the data shared (and unshared) points to a few scientific issues
1. It proves the sci that the election is a super-spreader event
2. Some returnees/politicians (no data shared) cud be super-spreader persons thestar.com.my/news/nation/20…
3. The massive surge of cases in Sabah reinforces this super-spreading phenomenon of the elections
4. The 30.8% Sabah cluster in Klang Valley could have been prevented if MOH: a)quarantined the returnees/politicians from #Sabah (b)re-tested the returnees
5. @DrAmarMOH showed down-trending of unlinked cases fr 63% to 7% (12 Oct). Shows that Klang Valley outbreak was clustered. This is “relatively good” news since clusters are easier to detect-track-contain. It debunks need for CMCO if rapid testing-tracing-isolation was robust!

They go unreported-out of sight out of mind!
PH funding < Sec/Tertiary/Curative/Tehab
eg Success of Vaccines-Took us 15 years to get PCV in our Nat Imm Program in 2020
But 1 case of polio/diphtheria-Msia goes hysterical!
2/ To detect #COVID19 before it spreads, PH needs FREQUENT/ACCESSIBLE/RAPID TESTING
We cannot detect pre/asymptomatic cases before they spread if we do not test FREQUENTLY
RTK-Ag picks out the infectious cases.
