The Equal Opportunities Form in your job application has 'gender' in a list of what appears to be the protected characteristics under the Equality Act 2010.

You then say that your Equal Opportunities monitoring were "developed with the support" of @ehrc and "follow what was used in 2011 census."
However, the @ehrc recommend collecting data on the protected characteristics under the Equality Act 2010 and 'gender' is not a protected characteristic.

The 2011 Census asked "What is your sex?" with options:

Male
Female.

It did not ask for 'gender'.
Can you explain these discrepancies?

You don't say what you mean by 'gender identity' but female and male are terms for the two sexes and are not 'identities' so your question is confusing.

It's also not clear what you mean by the other terms you use.
