“I’m so burned out. I can’t do this anymore.”

This is one of the most common issues I address with my #physicancoaching clients.

We avoid #burnout and increase/maintain our #resilience when our basic needs are met.

Let’s start with the basics.

A thread 🧵:
Physician burnout is largely due to organizational culture and operations.

Personal resilience plays a smaller role.

Physicians are more resilient than the average adult population.

Yet, resilience training is often promoted as the primary way to prevent burnout.

Yoga and meditation are great (I do both), but they are insufficient interventions for burnout when our basic needs are not met.

Consider burnout recovery and resilience based on Maslow’s hierarchy:

Physical and mental health:

I am hydrated.
I have access and time to consume nutritious foods.
I am rested.
I have time/access to restrooms.
I can easily access mental health resources.
I do not cope by substance use.
I have time/access to private space to breastfeed/pump.
Safety and security:

OSHA estimates that workplace violence is 4x higher in healthcare than private industry.
I feel safe at work.
My patients are safe in my workplace.
My job is secure.
I am compensated fairly.
Respect:

I feel valued, included, and respected by my administrators, colleagues, and patients.
My workplace, colleagues, and patients do not discriminate
My family time is respected
I had adequate maternity/paternity leave.
My EHR is efficient.
Equipment and processes work well
Appreciation and Connection:

I am noticed and appreciated.
I feel connected to my patients and colleagues.
My compensation reflects appreciation.
I respect my leadership team.
Healing patients and contributing to the fullest of one’s ability:

I have time, autonomy, and resources to heal patients.
I have time to think and contribute.
I can fully engage in teaching my students/team.
I am living in alignment with my purpose.
How are your basic needs being met?

What is ONE need that you can meet in the next month?

Who/how can you ask for help?

What would you add to this list?
