"Grave incident" has happened in North Korea due to a lapse in anti-COVID measures says North Korean state media. (Yonhap)
Kim Jong-un said via state media that a "grave incident" had happened that could threaten the safety of his people and country. Kim accused senior officials of neglecting their duties in carrying out measures important to thwarting the spread of COVID-19.
Neither KCNA or Kim Jong-un himself elaborated on what the "grave incident" was.
North Korea has maintained for months now that it was COVID-19 free. Obviously something substantial has occurred for Kim Jong-un to talk in such a bleak way like this.
It's not known exactly what Kim Jong-un is referring to, and we may never know, but I have a sneaking suspicion there's some sort of widespread COVID-19 outbreak in North Korea currently. On top of horrific food shortages, times are testy for the regime.
Per @chadocl, North Korea held a politiburo meeting on June 29th to deal with the "negligence" amongst the senior officials. KCNA says there was a "major event" that caused a "great crisis" and there were serious consequences caused by it.
For a country in the midst of a "grave" COVID-19 related incident, there's a definite lack of masking and social distancing here amongst officials.

China: The U.S. gov't is assessing a reported leak at a Chinese nuclear facility after a French company that partially owns and operates the facility warned of an "imminent radiological threat". Biden administration believes the facility is "not yet at crisis level", per CNN.
Warning by the French company also included an accusation that Chinese authorities were raising the acceptable limits for radiation detection outside of Taishan Nuclear Power Plant in Guangdong province to avoid having to close the plant.
Nonetheless, the situation is important enough that the U.S. National Security Council held multiple meetings regarding the topic last week as they monitored the situation. Meetings were deputy level & assistant secretary level.
