Vague and Non-Specific Cancer Symptoms: A short Tweetorial.
A bit of context courtesy of #GP150w for the #blackandwhitechallenge faced in Primary Care: 1/7
Vague and Non-Specific Cancer Symptoms
Continuing the shades of grey, we differentiate the specific cancer signatures from those non-specific symptoms, noting the lowering of the threshold for Positive Predictive Value to the current 3% level. 2/7
Vague and Non-specific Cancer Symptoms
So, what does the evidence for new pathways say?
Quite alot of support for 'gut instinct' or Clinical Suspicion- one of the two commonest reasons for referral to Multi-Diagnostic Centres, and statistically significance to support it. 3/7
Vague and Non-specific Cancer Symptoms
A 2021 @BJGPjournal review by the Lead Author has suggested that given the degree of less common cancers diagnosed via MDCs that this pathway exceeds that 3% PPV threshold- opportunity for hope! 4/7
Vague and Non-specific Cancer Symptoms
A useful moment to stop and highlight the data from this @BJGPjournal Systematic Review from Friedemann Smith on how valuable listening to that inner voice is! bjgp.org/content/70/698… 5/7
Vague and Non-specific Cancer Symptoms
Considering Unexpected Weight Loss:
Helpful evidence from @BrianDNicholson that we don't need to investigate all cases through these pathways, but associations with other features/symptoms should give you the nudge. #PracticeChanging 6/7
Vague and Non-Specific Cancer Symptoms
We're all set to launch our local pathway, but how much will the pandemic affect the 1st presentation of the patient?
Will we see more presenting at late stages and less at the vague or non-specific? @CRUKHCPs@dottietipsy@_JamesGwilt 7/7
A Sunday 20-tweet thread:
The RCGP Member Survey on #PhysicianAssociates gathered the views of 5,112 members- around 10% of Membership, 22.04.24-13.05.24.
The results are published both in a report on the RCGP site, with data tables ready for download. Real transparency. 1/20
45% Male, 53% female proportions of AiTs (GP Registrars) & Members/Fellows of the College in line with the total membership. 84% of respondents practice in🏴 7% 🏴 4% 🏴 & 2% NI.
Within Members/Fellows 26% Partners, 24% Salaried, just 10% Locums & 1% working in Out of Hours. 2/20
Of those working in a practice, 40% of us are serving between 10-20k patients. ‘Does not apply’ should be at least 10% if that’s the proportion of locums, but maybe they have a role for part of the week and locum as well. 44% of respondents have never worked with PAs in GP. 3/20
PSR breach came from interest payments on the stadium build, not player transfers. The rule on this was changed mid-process. No other club was stadium-building when this change was made.

The commission acknowledged there was ***no sporting advantage*** due to the breach- it wasn’t done deliberately or cynicallly.
Mitigating circumstances haven’t similarly affected others like interest rate rises have: Income via Alisher Usmanov USM & MegaFon withheld due to Russia’s invasion of Ukraine. This affected player trading to put Everton in need of revenue streams

In case this helps for getting a flight refund:
In July last year our holiday flights were cancelled.
We booked through an online holiday company & their chosen airline for the break. Both extremely difficult to speak to directly; all online chat help
We got our refund today 👀
Our money was paid in instalments to the holiday company and we got our ATOL Certificate, breaking down the flight & hotel costs.
We were checking on the flight details regularly in the lead up to travelling? And 8 days before holiday flight was cancelled; airline didn’t call us
Seeing the cancellation we had to call the airline’s premium rate phone line; that was a 50-minute wait to confirm the cancellation.
No alternatives offered.
Refund should take 15 working days.
(It took a lot longer…)
Forked out extra money on the brilliant @jet2tweets & all ok

It’s Semi-Final day in the 150th year of the FA Cup #FACup150
Although it’s the 140th competition due to 10 years without the Cup during the wars…
Most Semi-Finals:
3️⃣0️⃣ #Arsenal & #ManchesterUnited
and fittingly neither of them are there this weekend 1/8 🧵

It’s Semi-Final day in the 150th year of the FA Cup #FACup150
It’s Liverpool’s 25th Semi-Final today; (21 post-war)
The Reds have won their last FIVE in this competition (92, 96, 2001, 2006, 2012). 10yrs since their last semi is the longest gap since 1963 #MCILIV
#EmiratesFACup
It’s Semi-Final day in the 150th year of the FA Cup #FACup150
It’s Manchester City’s 17th Semi and remarkably they’ve only lost FIVE, winning 8 on the trot between 1933-2013.
Less good news:
They’ve lost 3 of their last 4 before today: 2017, 2020 & 2021. #MCILIV
