At least it provides the clinicians and patients with some estimates of the underlying probability with the available information @ibddoctor #MNIBD#GITwitter#Medtwitter
1/n Tweetorial on Checklist for advanced therapies for inflammatory bowel disease
Here is what I summarized. Credits to @Bealoquebea and @avstmd as I used some of their tweets for this presentation. I am sure there are areas of uncertainty, please do share your thought
I am sure we have read this book. Personally this book converted me into a believer of checklists
Although the term itself is probably being challenged, advanced therapies usually refers to biologics and small molecules
Anything beyond 5-aminosalicylates, steroids, and immunomodulators
Recently I had an opportunity to speak on Anemia in IBD at the IBD Conference in Delhi organised by @CCF_IN and APAGE. Here is a tweetorial focused on Iron deficiency anemia in IBD
Anemia is common in Inflammatory Bowel disease, most frequently due to Iron deficiency.
Battling disease activity, diagnosis and management of anemia often take a back seat
Key msg- Screen for anemia at the time of IBD Diagnosis doi.org/10.1097/01.mib…
Quite often, underlying causes are multiple
- Ongoing bleeding
- Restrictive diets
-Reduced absorption
- Anemia of Chronic inflammation due to functional iron deficiency
- Drug-induced
Here is a tweetorial on Antitubercular therapy (ATT)-related hepatitis. This is based mainly on a lecture I took for DNB residents
Here we begin
1/n⤵️
Three of the four first-line drugs are potentially hepatotoxic
Many of the second-line drugs are also recognized to cause hepatotoxicity
Those in blue font are potential hepatotoxins
Metabolism of Isoniazid is complex and many enzymes play a role- polymorphisms may potentially explain, in part, why some of the patients suffer from hepatotoxic effects of INH
As health professionals it is our duty to correct misinformation but please remember most of us are tired and drained. It is not easy to stand up to and fight the pandemic of misinformation in each of the residential and family groups
We should always share responsibly but the responsibility increases manifold during these torrid times.
This is something a well intentioned citizen shared on whatsapp group of our society.
Not only it promotes an unknown medicine but argues against 'running' after oxygen
When questioned about evidence he shared another image
2/nMicrobiological diagnosis of ITB is possible in only 20-40% cases. Discriminating Intestinal TB and Crohn disease is tough and most clinicians in endemic countries encounter this confusion commonly 😕. . #Tuberculosis
3/n Then how to differrentiate: When in doubt, we start ATT. We know it could be Crohn's but the consequences of starting immune-suppressing treatment in Intestinal TB could be disastrous. tandfonline.com/doi/full/10.10…
