Why was the name changed? Because Kerala has the hidden savarna mindset.
The director-actor understood it. The arrival of a Savarna male as an intelligent hero is considered natural and while a Savarna male as a cruel villain (Puzhu), who is also casteist, is deemed unnatural.
Puzhu perfectly portrays the paranoia of Brahmin men and clearly, it has rattled the conservative Right-wing (RW) of Kerala. The movie broke the conventional path that Malayalam cinema has taken since the 1950s and got a backlash.

Why was Kammatipaadam attacked?
Kammatipaadam was a rare Malayalam movie where both the protagonists are Bahujans. Krishnan (Dulquer) is an Ezhava boy who migrates from Alappuzha to Kochi and Ganga (Vinayakan) is a Dalit who grew up in the paddy fields of Kammatipaadam.
The Movie narrates how the Dalit community was forced to give up their lands to the Real estate Mafia. Ernakulam was a small town during the early 1950s and small tracts of farmland were given to all landless communities, mainly the Dalit community, under the Land Reforms Act.
Krishnan tells the transformation of Cochin (Ernakulam) from its green past of open paddy fields to a metropolitan city with skyscrapers in the mid-1990s through a cycle of bloodshed and violence.
The water-rich agricultural land, which was owned by Dalits, was forcefully taken away and they got no compensation in return.
Kammatipaadam speaks of countless Indian cities that are built on top of the blood & tears of the underprivileged masses.
And the movie rattled many.
Kerala is an enigma. It is the most developed place in South Asia in terms of HDI but it still hides Patriarchy & Caste in a subtle manner. And when exposed, the conservatives get angry.
