We @UKHSA have updated our monkeypox #MPX case counts to 106 confirmed cases in the UK.

A few things have changed over the past two weeks. A short explainer thread here.

Following initial reports of cases in gay men, we have seen a large increase in reported cases in this group, also including bisexual men, and other men who have sex with men.

We know this outbreak is being transmitted in networks that are connected through sexual activity.
It probably has in the past, given that transmission between people who live together has been documented extensively.

The above does not mean that MPX is not, and can not, be transmitted through other networks, or routes.
This transmission pattern is happening because the virus spreads through close, direct contact — sex just happens to be one of the most popular forms of direct contact. And it is absolutely expected for people of the same sexuality to have sex with one another.
Infections do not have sexual orientations.

With the right knowledge, people can be equipped to make their own decisions, and take care of their health.

There is an army of people across @UKHSA @BASHH_UK @NHSuk working to control this outbreak. And there are a few things you can do if you are think you might be at risk.
First, we are asking you to be hypervigilant of any rashes or lesions you might see in your skin, including face, hands, and genitals. If you do have any of these, call your local sexual health service. You might need to come in for a test.
Second, we are doing our best to manage cases and their contacts. Inevitably this involves establishing a link with your sexual partners.

Please do your best effort to obtain contact details of any new or casual sexual partners, such as those met through Grindr or in parties.
Third, you can choose to change your sexual behaviour, such as reducing the number of partners you have, or only having sex with people you know you could contact. It will be your own personal choice. You have information, freedom and control.
As individuals we all balance our risk and pleasure everyday, but I do think we can control this outbreak if we come together as a community.

We are already disproportionately burdened by infections. I think we can choose together not to have another one.

Yesterday, @UKHSA reported four cases of monkeypox (MPX) in gay and bisexual men in England, making a total of 7 cases nationally.

So what's going on with monkeypox (MPX)?

MPX is an infection usually acquired via contact with infected animals from rainforests. It's rare, but eventual outbreaks of human cases in West and Central Africa do occur.
When MPX jumps from animal reservoirs to people, the spread from person to person is usually limited: some of the longest transmission chains documented include six successive person-to-person infections.

En Estocolmo, visitando a mi familia, nos cruzamos en la noche de año nuevo con una vecina que nos saludó amable y en plena conversación nos dijo que estaba en ese mismísimo momento con COVID.
Explicó que no salía hace un par de días, pero que cómo no iba a salir a caminar a ver el año nuevo y los fuegos artificiales. No usaba mascarilla, y llevaba a su lado a su hija, recuperada de contagiarse en el colegio.
Si bien considero que el uso de mascarillas en exteriores no es necesario, ver a alguien sintiéndose cómodo de salir a dar un paseo sin ella, estando con COVID, me dejó descolgado.

Van saliendo resultados sobre severidad de las olas de #Omicron, y todos sugieren que la infección por esta variante en poblaciones vacunadas o con exposición previa al virus causa menos enfermedad severa, hospitalizaciones y muerte. (1)
Sin embargo, de ahí a afirmar que “es ideal que todos nos infectemos con Omicron” hay un salto lógico muy grande que hay que atender. (2)
La severidad es el resultado de la interacción entre el virus y nuestro sistema inmune, pero no es una característica intrínseca del virus. Cuando hablamos de “severidad” es importante diferenciar dos conceptos: “virulencia” y “susceptibilidad”. (3)

Cuando aparece una nueva variante, hay tres dimensiones o características que deben determinarse:

(1) Transmisibilidad
(2) Escape inmunológico
(3) Severidad

Aquí explico un poco para saber qué esperar en las siguientes semanas.
La transmisibilidad es la capacidad del virus de pasar de una persona a otra. Se puede estimar por el valor de R0.

Para medir la transmisibilidad, se pueden usar principalmente dos tipos de estudios.
Los primeros son estudios en escenarios "cerrados" como un avión, un bus o un crucero, en donde podemos medir la "tasa de ataque" o la cantidad de casos secundarios a partir de un solo caso primario.

Muy importante para el Perú. Toda la evidencia hasta ahora sugiere que variante #P1 (Brasil-Manaos) de #COVID19 tiene mayor transmisibilidad y escape inmunológico.

Esta es una VOC - variant of concern (de las más amenazantes)
El aumento en transmisibilidad es importante porque, al transmitirse más fácil y rápido, la cantidad de casos puede aumentar de forma muy súbita haciendo presión adicional sobre el sistema de salud. Se estima que se transmite al menos el doble de fácil.
El escape inmunológico implica que las personas con anticuerpos (ya sea por vacunación o por infección previa) tienen riesgo de contraer el virus. Quizás no enferman de forma severa, pero pueden contribuir a cadenas de transmisión. Ahí perdemos una defensa adicional.
