Also - we (the authors) paid out of pocket to publish this piece on @JAMANetworkOpen so that it would not be behind a paywall. We need more resources (aka funds!) for open academic research 🙏🏽
Documentation burden directly impacts #burnout for clinicians as well and it does so disproportionately between disciplines & gender
This work highlights what we (clinicians) know to be true. There is too much Info in the #EHR (& it’s growing) to be sustainable
Things must change
No surprise that 70% of errors are related to comm failures
This why I dedicated 15 yrs to find new ways to communicate & work as a team. @CareAlignAI (spin out of @PennMedicine which started as my resident QI project & of which I am now CEO) is a wiki-style, iterative, collaborative documentation platform.
My goal is to make it easier for clinicians to do the right thing - that includes better documentation, better communication, more joy in medicine ❤️
Can you read hamlet x2, in 40 word pages, each on a separate web page, where 20 words on each are copied but u don’t which ones?
And then go make life & death decisions based on that info?
That’s what clinicians are asked to do regularly
Supporting Data:
Hamlet (Shakespeare’s longest written work) is ~ 29500
The median chart length in our corpus was ~4280 words (1/6 of Hamlet)
12 charts in one day = Hamlet twice
Ave note length = 40 words
50.1% duplicated
For those of you asking / interested in learning more about @CareAlignAI, we are dong a webinar with @drsanders of IMO tomorrow on how #documentation & coding do not have to be at odds. We can design tech to make documenting AND #coding easier
