I don’t think viral replication will be the only way COVID-19 can make you go blind. I think clots will still play a part, whether in the retinal vein/artery or in the brain.
The eye is an example of an “immune privileged site” like the brain and testes.
Infection (in general not just C19) can persist in these locations for a long time…->
-> and be difficult to treat because drugs don’t get delivered to these locations as easily.
Infection of immune privileged sites can also lead to inflammation that causes leakage of immune cells and antibodies to that site. This can cause the body to recognise parts of the body as “foreign”. ->
Autoimmune attack of eyes/brain/testes can occur (again in general, not only C19).
So I think the eye is at risk from all three mechanisms.
Infection.
Clotting.
Autoimmune destruction.
The vision loss may be gradual or sudden.
As for how to prevent COVID-19 eye infection.
I am waiting for the results of some mechanistic studies on what happens when you expose eyes to virus.
I still wear eye protection in high risk scenarios but->

I want to say something that will wind up all my followers.
Firstly, I don’t encourage taking risks with COVID, and if I did take risks I certainly wouldn’t post them online because I know what the effect might be in terms of role modelling… BUT…
You all know that two people that mask with a well fitted respirator, adjust their environment with HEPA or fresh air 99% of the time, and generally live a low risk COVID lifestyle…if those two people decide to have one moment outdoors and unmasked with eachother…it’s low risk.
I think this is particularly important for young people.
If you spend your time mostly protected at work/Uni/supermarket/cinema etc masked… and you take your mask off to be with someone else that lives that lifestyle… it’s not no risk, but it’s lower risk, and that’s worth it.
This is very heavy stuff. Realistically, at some point I think I will get COVID-19, unless I die of something else first. So…
knowing the information that I do, about the risk of many different kinds of problems, I have to work hard to maintain equilibrium of some sort.
My tactic is to take the appropriate steps to reduce the risks, advocate for appropriate change, fulfil my role in life by trying to set a reasonable example and beyond that… I don’t think about it.
Of the COVID careful amongst us… are you by any chance exhausted due to overwork caused by everyone else at work getting COVID?
Do you get called day and night to cover people?
I know covering sick leave is normal, but when you are the only one left on the roster it gets very, very silly.
The worst of it is… you are the one that is absolutely exhausted due to work, and they are all absolutely exhausted because they are convalescent… and they still make you sit through all their stories of the fun they had getting sick!
Next time I accidentally set something on fire, instead of reaching for the fire blanket, I’m going to have a family meeting and vote to end the cooking disaster.
