Urgent CT๐Ÿ‘‡๐Ÿป

Husband reveals she was on blood thinners after her โ™ฅ๏ธ surgery, 5 yrs ago ๐Ÿ˜ฑ

How do you manage this patient?

Read this thread to know more ๐Ÿ‘‡๐Ÿป

1/12 Image
Yes, the CT shows a large hematoma

The patient was most likely on a vitamin K antagonist (VKA) post her prosthetic valve surgery!

The labs are back, she has also started to ๐Ÿฉธ through Foley's catheter๐Ÿ˜ญ

โ€ขCBC: 8/5600/201k
โ€ขINR: 8.8
โ€ขaPTT: 32s (control 29s)

This is a classic example of warfarin toxicity & unfortunately quite commonly seen here in ๐Ÿ‡ฎ๐Ÿ‡ณ

Why ?

The therapeutic window is ๐Ÿ‘‡๐Ÿป

#MedTwitter

3/12
Also, a lot of drugs/food interact with VKA ๐Ÿ”„

I always ask patients about recent antibiotics/over the counter/herbal products !

Antibiotics are the most common culprit and every tom, dick and harry gets amox-clav in ๐Ÿ‡ฎ๐Ÿ‡ณ

#medtwitter

4/12 Image
Our patient also took some antibiotic a couple of days back and now this ๐Ÿ˜ญ

How do we manage this now ?

#medtwitter

5/12
Call neurosurgery, STAT ๐Ÿ“žโ˜Ž๏ธ

Start supportive care, STAT

Anti-oedema measures, STAT

Protection of airway, breathing, circulation as always

STOP WARFARIN (obviously)

And something more ๐Ÿ‘‡๐Ÿป

6/12
How do we reverse the anticoagulant effect that's already present ??

Many options:
1. 4 factor PCC
2. Recombinant factor 7a
3. FFP
4. Vitamin K

She should definitely get 10mg of vitamin K iv, STAT ๐Ÿ‘๐Ÿป

#medtwitter

7/12 Image
What's 4 factor PCC ??

It's a pathogen-reduced, lyophilized concentrate of 4 clotting factors:

Factor II
Factor VII
Factor IX
Factor X

These are the factors the VKA have depleted, so we replace them ๐Ÿ‘๐Ÿป

But why not FFP?

#medtwitter

8/12 Image
Simple, FFP = more volume
FFP = less concentrated w/ factors
FFP = chance of blood borne infections

So FFP only if If PCC isn't available ๐Ÿ™๐Ÿป

How much FFP ?

Give 2 unitsโžก๏ธmeasure INRโžก๏ธstill >1.5โžก๏ธgive 2 more units and so on !!

#medtwitter

9/12 Image
We have PCC in the hospital, what's the dose used ??

2000 units iv over 10minutes
โฌ‡๏ธ
Check INR after 20-30min
โฌ‡๏ธ
If >1.5
โฌ‡๏ธ
Repeat the dose again (50units/kg)

#medtwitter

10/12
Husband asks, how's the prognosis??

This is THE MOST FEARED COMPLICATION of warfarin.

Outcomes, unfortunately not so good.

The larger the bleed, poorer the outcome.

Early reversal of AC and rapid neurosurgical evaluation are a must

jamanetwork.com/journals/jamanโ€ฆ

11/12
Summary:

Warfarin- narrow therapeutic index
Lots of interactions
Intracranial hemorrhage is ๐Ÿ˜ญ
Rapid reversal necessary pre surgery
PCC>FFP
Vitamin Kโœ…

Outcomes remain poor, best to prevent this complication. Adequate councelling at the time of initiation is important ๐Ÿ™๐Ÿป

End.
As always ๐Ÿ‘‡๐Ÿป

Hey #MedTwitter ๐Ÿ‘‹๐Ÿผ

A bit about SMUDGE CELLS today!

Also called smear cells because they were initially thought to be artifacts of smear production!

Also called Gumpretch cells after Dr. Gumpretch who first described them in the 1800's

But there's more ๐Ÿ‘‡๐Ÿผ

#MedTwitterImage
Where do you see them?

Many conditions, but it is almost always seen in patients with Chronic Lymphocytic Leukemia (CLL) ๐Ÿ‘‡๐Ÿผ

#MedTwitterImage
Some patients have very few smudge cells while others have an abundance of these cells ๐Ÿคท๐Ÿผ

Why??

As a physician I'm sure you've seen patients with ITP!

Let's start by calling it IMMUNE thrombocytopenia instead of IDIOPATHIC thrombocytopenia.

The P stood for Purpura in the past. Not anymore!

T= Thrombocytopenia = <100,000 platelets

#MedTwitter

ITP comes in 2 flavours:

โ€ขPRIMARY
and
โ€ขSECONDARY

SECONDARY TO WHAT?
Long list of possible causes, autoimmune disease, HIV, HepC, drugs etc

#MedTwitter

2/20
If the thrombocytopenia lasts >12 months, the patient has chronic ITP ๐Ÿ˜ญ

This is unfortunately more common in adults; almost 75% develop chronic ITP ๐Ÿ˜ญ

Hey #MedStudentTwitter ๐Ÿ‘‹๐Ÿผ

Do you know what this image shows?

Obviously you do !!

If not, read this short thread ๐Ÿ‘‡๐Ÿป

Yeah, those are hemighost cells !

That's a hallmark of severe oxidative damage to red cells. Most often seen in G6PD deficient individuals!

I say most often because it can happen in patients with normal G6PD levels when the oxidative damage is severe ๐Ÿ˜ญ

#MedTwitter

You cannot see Heinz bodies in that image ๐Ÿ˜” They will NOT be seen on Romanowsky stains !!!

The red cell will appear like an emptied out "ghost cell" when the hemoglobin denatures ๐Ÿ˜” That's what you saw โ˜๐Ÿผ

๐Ÿ‘‡๐Ÿป Is a supravital stain showing Heinz bodies (blue dots)!

Now let's see what each spike represents!

ฮฑ1 =ฮฑ1 antitrypsin +others (small amount)

ฮฑ1 antitrypsin (AT) is an ACUTE PHASE REACTANT, so it'll โคด๏ธin INFLAMMATION

Obviously, ฮฑ1 isโคต๏ธin ฮฑ1 AT deficiency!

It's quite simple, isn't it?

Wait, why is ฮฑ2 โคด๏ธ in acute inflammation๐Ÿค”

2/16
Image
Image
To answer that, we need to know what ฮฑ2 contains !

Since it's elevated in acute inflammation, it must be an inflammatory marker ๐Ÿคท๐Ÿปโ€โ™‚๏ธ

Yes, it is ๐Ÿ™Œ๐Ÿป

ฮฑ2 contains ceruloplasmin, haptoglobin and ฮฑ2 macroglobulin, all acute phase reactants!

Now we know why we get ๐Ÿ‘‡๐Ÿผ pattern !

Hey #medicine residents !!

Time to talk about EOSINOPHILS today. Not as common a consult as thrombocytopenia and anemia but important nonetheless.

A short ๐Ÿงต to touch upon all things eosinophilia ๐Ÿ‘‡๐Ÿป

#MedTwitter Image
Before we go any further, it's important to know this

๐ŸฉธEosinophil โ‰ฅ 500 is ABNORMAL

Eosinophil is a predominantly tissue dwelling cell so ๐Ÿฉธ eosinophil DON'T correlate with tissue damage ๐Ÿคทโ€โ™‚๏ธ

We see a few patients with โคด๏ธโคด๏ธEos and no ๐Ÿซ€๐Ÿซ injury !!
Where do eosinophils come from?

Short answer: Bone Marrow

They are cells of the granulocytic lineage, siblings of neutrophils and basophils, children of the myeloblast (image๐Ÿ‘‡๐Ÿป)

IL5 is the cytokine responsible for it's production, important to know because we have anti IL5 mAb Image
