Had an interesting case recently which left me with a lot of things to ponder on!

A 50yr old Male, brother of a sanitary worker of our ward, a worker himself visited the OPD with complaints of increased fatigue & shortness of breath even with mild exertion since a month.

1/N
He had previously visited the Medicine OPD and was quickly referred to the Cardiology Department suspecting a heart issue and on patients demand. Entire cardiac consultation had come out to be normal. BP, ECG, TpI, BNP, CXR and 2D ECHO - no abnormality was detected. 2/

Got some time to read up a bit. So here we go -

Sustained Monomorphic Ventricular Tachycardia

CF as described in my case - hypotension, syncope & some might even be able to maintain BP
Usually VT with HR > 200 - Hypotension
<150 - can be compensated unlesd LV dysfunction
1/n Pathophysio - usually focus is an area of fibrosis due to old infarct/inflammation or prior cardiac surgery
Rarely reentry in a diseased purkinje system.

Mechanism - Re entry circuits

1. SVT with LBBB or RBBB
2. SVT with an accessory pathway
