While masking is critical- there is nuance because masking *outdoors* will be largely offset if people then do not mask *indoors*, when transmission is highest. Need to ensure masking is happening in highest transmission risk situations; I am most worried about in the home.
2/ Clusters of home-based cases were significant in early Wuhan reports. I found the same to be true when I took care of #covid19 patients in April. Vast majority seemed to be infected by a family member in the home, often before symptoms had started.
3/ Tough problem without a clear solution.

Masking consistently outside the home will make one less likely to contract the virus; but if one gets sick, transmission indoors in the home while presymptomatic seems like a high-risk situation.
4/ This is compounded even further in homes that are crowded, which likely overlaps w/ being poorer/ working frontline jobs that put the breadwinner at higher risk to get #covid19 to begin with. This is why PPE for these service workers is beyond critical.
5/ This is likely a substantial part of the story in this country. Ask doctors like @RanuDhillon who are seeing cases right now in Northern California; I saw the same in Boston in April/May. This has everything to do w/ the social determinants of health.
6/ This is also why isolation options *outside* the home are a *must* especially for workers who can’t safely isolate at home without putting their families at substantial risk. Need to ensure safe isolation when transmission risk is highest early in the disease.
7/ The example of Wuhan is notable. While some call it “extreme”...they made a big effort to ensure no active infection returned home where vulnerable family members were.

You waited in clinic for your PCR result; if it was positive, you went to an external isolation center.
