TB granulomas are large and confluent, may show necrosis.
AFB + confirms the diagnosis

Here is a foreign body (food particle) granuloma in peritoneal aspect secondary to rupture.

Sometimes we see granulomas along advancing margin of colon cancer. AFB is usually negative.

A case of CD with small ill defined granulomas. Note the location away from crypts.
