The WMSC, WMATA and coupler manufacturer participated in a detailed review of the coupler assembly from last week’s Red Line train pull-apart near Glenmont & maintenance documentation.
Investigation remains in its early stages, but the first week has identified a number of facts:
There were five loose bolts on the top of the coupler assembly.
One, the “clamping bolt” or “pinch bolt,” is designed to hold the draft bar in place.
The other four bolts are intended to hold a guide rail in place that can help prevent the coupler assembly from rotating.
The screw-like threads on the draft bar and coupler head were worn or damaged.
Corrosion and contaminants were found on the threads of the coupler head.
WMATA does not have the tools required to check the threads on the draft bar and coupler head during an overhaul process.
Parts of this coupler were included in WMATA’s fleet-wide inspection of 6000-Series cars following the Oct. 9 pull apart.
The gland nut on the coupler involved in the Nov. 24 pull apart had a properly aligned torque stripe.
Car 6177 last underwent regular maintenance on Oct. 21
Metrorail personnel followed the proper chain of custody procedures prior to this week’s reviews of the couplers involved in the Nov. 24 pull apart.

WMSC experts are on scene at Silver Spring (with social distancing protocols) where at least one car of a #WMATA Red Line train toward Glenmont derailed around 11:20 a.m.
According to the preliminary required formal notification to us from #WMATA (so pending further investigation), the eight-car train appears to have passed a red signal prior to the second car derailing. No rider injuries
The WMSC’s focus is to determine all the causal factors that led to the accident. The investigation includes photos and measurements of the scene, any damage to railcars, tracks or switches - and completion of that documentation prior to re-railing
