Good summary on the efficacy of the 6 reported Phase 3 vaccines by the ever wonderful @MonicaGandhi9. Though caveats on #B1351 variant:

📍Moderna & Pfizer efficacy unknown for 🇿🇦; lab shows lower neutralization.

📍Sputnik not tested 🇿🇦

📍New AZ data shows low 22% #COVID19
2) to be clear, the 22% efficacy against mild disease for the #B1351 variant was not significant in the new Oxford-AstraZeneca trial report. For severe diseases, it was okay for neutralization but no efficacy data yet. SA has switched vaccines they will be using meantime.
3) The South Africa 🇿🇦 variant has very evasive properties of seemingly evading prior immunity from natural infection. Prior infection was not protective against #B1351- not even for severe disease. But fortunately, several vaccines do work them n
4) The South Africa 🇿🇦 #B1351 fighting efficacy of AstraZeneca vaccine was indeed poor, 22% and not significant for mild and moderate infection. But... that wasn’t true before B1351 emerged.
5) early on in the Oxford-AstraZeneca trial, before B1351 became dominant, the vaccine had an efficacy of 75% risk reduction!!! But all that disappeared once B1351 variant showed up!

💡Hopeful—ASTHMA STEROID INHALER—promising trial found early use of inhaled budesonide (usually for asthma) reduced risk of needing urgent medical care & reduced time to #COVID19 recovery by ~1 day! Also treat 8 patients➡️prevent 1 deterioration—amazing! 🧵medrxiv.org/content/10.110… Image
2) Clinical recovery was 1 day shorter in the budesonide arm compared to the usual care arm (median of 7 days versus 8 days respectively, p=0.007). Proportion of days with a fever and proportion of participants with at least 1 day of fever was lower in the budesonide arm.
3) Fewer participants randomised to budesonide had persistent symptoms at day 14 and day 28 compared to participants receiving usual care.

Lyme Disease might be actually a chronic infection, not just an autoimmune response, says new book “Chronic: The Hidden Cause of the Autoimmune Pandemic and How to Get Better Again,” by Dr @StevePhillipsMD, a Lyme practitioner & researcher & @danaparish. nytimes.com/2021/02/02/opi…
2) the spread of what the authors call Lyme+, an array of tick-borne pathogens that often infect patients simultaneously, is responsible not just for over 400,000 cases of Lyme disease diagnosed each year in US but also for an unknown number of chronic infections beyond that
3) Chronic is a great book. I’ve had many family who had such chronic conditions. It’s certainly an insightful look. nypost.com/article/best-n…

Apple Watch predicted #COVID19 in healthcare workers ~1 week before positive PCR test confirmation, says new study from Mount Sinai researchers. 🧵
techcrunch.com/2021/02/09/mou…
2) dubbed the “Warrior Watch Study,” it used a dedicated Apple Watch and iPhone app and participants from Mount Sinai staff. It required participants to use the app for health data monitoring & collection, & fill out daily survey on potential COVID-19 symptoms and other factors.
3) During the study, the research team enlisted “several hundred healthcare workers” to participate, and collected data over several months, between April and September. The primary biometric signal that the study’s authors were watching was heart rate variability (HRV),

NEW VARIANT—New California variant CAL20C has recently found in over **half** of all samples in Los Angeles #COVID19 cases, & ~20-30% across CA. “I’m decently confident that this is a more infectious strain of the virus”, says expert from Cedar-Sinai. 🧵

2) Dr. Charles Chiu, a virologist at UCSF said “It just popped up under our noses, and now it’s rising in multiple counties,” he said. “On the whole, it’s safe to say it’s going to spread outside of California.”
3) “Researchers are also looking in other states for CAL.20C, Dr. Plummer said, and have so far found it in Arizona, Connecticut, Maryland, New Mexico, Nevada, New York, Texas, Utah, Washington, Wyoming, & DC. It’s not clear yet how common it is outside California.”

💡SOCIAL MURDER—Scathing BMJ editorial—“At the very least, #COVID19 might be classified as “social murder,” says two professors of criminology. Today, “social murder” may describe the lack of political attention to social determinants and inequities that exacerbate the pandemic.” Image
3) “How big an omission is not acting immediately after the World Health Organization declared a public health emergency of international concern on 30 January 2020?”

📍Outbreak of #B117 despite full vaccinations at a nursing home. 14 residents at a 🇩🇪 nursing home have tested positive for 🇬🇧 B117 variant, despite all having 2 doses of the BioNTech-Pfizer #COVID19 vaccine—last shot on Jan 25. All mild.

➡️Keep masking. dw.com/en/coronavirus…
2) This situation highlights the needs to continue to wear masks. Transmission vs protection against severe disease clearly are two different things.

Anyone refusing to mask because they say they have vaccine —remind them they still need to mask until pandemic is over! #COVID19
3) Granted, it takes 2 weeks to develop immunity, but they already had 1 dose with that lag time. And though it’s <2 weeks after 2nd dose, this highlights that protection is not complete, and even then it’s only 95% not 100%.
