Today during my on call I was asked to review the following case. The parents have given their consent to discuss via social media for educational purposes:

The case of the blue willied boy.

A thread 🧵
...The child in question was very upset and complained to his teachers that to his great surprise and distress that his willy was blue.
No other symptoms, no pain, going for wees without difficulty.
No fever, no cough and no change in smell or taste.
Sorry, force of habit. 🧵
So the understandably concerned teachers required a visual confirmation of the blue willied boy’s claims: cue call to parents, resulting in my involvement.
🧵
What was the cause?
Trauma?
Venous congestion or engorgement??
Torsion???
...using the mother who went to the school to attend we were able to carry out a sensitive visual inspection, with teaching staff awaiting the outcome.
Clearly an intimate area wouldn’t allow for remote visual inspection and the Safeguarding implications...
🧵
... the blue willied boy was successfully diagnosed and is now safely back home with parents.

But how, dear tweet-reader?
🧵
@threadreaderapp unroll please
Thanks for reading and commenting.
The kids are safely in bed, so I’ve got the time to explain.
I was called during my on call, but the child isn’t one of my patients.
I tagged in the #GParents I knew who may have insight of the problem of the blue-willied boy.
Parental consent to discuss the case was straightforward because the blue willied boy is my beautiful blue eyed boy.
He’s fast out asleep now, safe and well.
The clues were there in the thread 🧵
The emoji chosen as the cue for the clue
Intertwined, the thread within a thread. 🧵

The blue willied boy went to school with his lovely new blue pants.
After a bit of PE (note this isn’t a medical acronym!!) some blue fluff/dye attached itself to the little man’s little man.

A mummy cuddle and a paper towel fixed it.

From @TARGETabx toolkit on @rcgp website- prescribing of ABx has been creeping up since the turn of the century- it's a popular misconception that ABx were only overprescribed in the bad old days...! But there's been better news recently...
The response to this? #ESPAUR
