Long thread 1) There is this fear about the new Botswana/ Nu/ B1.1.159 variant but let's delve into why is this fear?? The 4 cases they found in Botswana were all vaccinated. So what??Breakthrough happens all the time,we underestimate incidence @hjelle_brian ctd)
Ctd2) This variant marked in blue is coming up , so what?? These variants will come and go. Ist it was the Wuhan strain in Grey , then the Beta ( SA strain in Green , then the Delta in Red and now may be the Botswana variant in Blue ( B1.1.529) @MoHFW_INDIA, @mansukhmandviya ctd)
Ctd3) For the virus and also the vaccines ( Pfizer/ Moderna/ AZ) the spike protein is the most important and that is where the maximum risk is . The Delta had 2 changes in spike proteins Receptor Binding Domain, the Beta had 3 while the Nu has 10 , so there is a real risk ctd)
Ctd4) The risk is the vaccine may no longer be efficient,so may be the Monoclonals that we have currently, also there is a real risk that the immune response may not be as good, simply put the variant may erase part of the immune memory that we gained from prior infection ctd)
Ctd5)But isnt that fear overhyped , probably not since there are some good people working on simulations and that these stimulations tell us that we need to preempt our responses ctd)
Ctd6) while there are voices to stop flights ,which we have raised as well , there is also a need to support Africa if we wish this variant to stay in Africa till we update vaccines. That means international support @WHO, @DrAxelrod_WHO, @AfricaCDC.
Ctd 13) Now this means that countries need to focus on both Public Health measures like masking with good masks and good quality air monitoring to make control possible, vaccines alone will be ineffective to control spread.
Ctd 14) while many of us think that the SA have done a great job there is now emerging information that we many have been already late because the SA consortium had a sampling bias. Now that 1 month hiatus maybe enough to seed the strain across the globe
Ctd19) Seems also that this mutant was able to do what the Delta was able to do in weeks .However there is a caveat statement that I wish to make , seemingly SA has very high incidence of HIV and that may have helped the mutant gain ground faster
Ctd20) If the South Africans have 20% incidence of HIV then we are looking at a global hotbed to ferment new variants avert.org/professionals/….
Ctd21) And Just heard that the US is awaiting for more information on the variant before deciding BUT the virus is not. It will enter the US if it hasn't already .Speed trump's over efficacy in a pandemic , if you wish to be perfect you will surely lose . arabianbusiness.com/videos/442939-…
Ctd23) while the UK thinks that they are smart to stop flights coming from SA and a couple of other countries, I think we may already be late .Sample this
Ctd24) The Mrna vaccines will take around 2- 3 weeks to get additional data and additional time to tweak their vaccines, then comes the difficult part of getting them into arms , with a variant like this one ( we hope to be wrong ) it will be late
Ctd 25) And that brings me to another important aspect, the value of holding fort & taking preemptive action ,while some want data to be sure they forget that it is the job of Public health officials to take decisions based on emerging data ,prevent a crisis before it happens
Ctd 26) Now we have some more concerning information , the patient from Belgium who came through Egypt was young and did not develop much symptoms till day 11, that means she should be in Cytokine storm BUT WÀIT at day 11 she still is throwing live virus like crazy ,concerning
Ctd27) Amidst all the gloom in the tweets above there was a great question by @hjelle_brian about is the virus changing too much and my answer is below. Agree we dont have a multi valent vax currently, but speed at which the virus has changed makes it possible in next 6 months
Seems that the US will do something about the situation. The United States will restrict travel for non-U.S. citizens from South Africa and seven other countries starting on Monday as a new Covid variant emerges.
President Joe Biden was briefed on Friday by White House chief medical advisor Dr. Anthony Fauci. In addition to South Africa, the affected countries are Botswana, Zimbabwe, Namibia, Lesotho, Eswatini, Mozambique and Malawi.

Will try and keep everyone posted on the new developments on Omicron , the last thread became too lengthy in a day .But this is the information we had on 25/ 26 Nov. Ctd
Ctd2) I believe that it has become imperative that we immediately start finding whether the combination of Merck's Molnupiravir &Pfizers Antiviral is effective against Omicron?? we have had reports of Molnupiravir dropping efficacy yesterday in full trial.@hjelle_brian ctd)
@hjelle_brian Ctd3) virologists consider that the virus is " stealing " parts of our RNA as @hjelle_brian, to me it seems that these are efforts of virus to get past Interferons which are our defence mechanism .We know that those with Anti Interferon antibodies have bad course #Omicron ctd)
Long Thread - With the new variants one from Botswana and the Second #C13 from South Africa , I am reminded of an old post @hjelle_brian ,@DrEricDing
For a variant to win against the Delta means a new Sumo wrestler is in town and that it has the potential ability to wipe out some of the herd immunity that we have achieved from the Delta .@PrinSciAdvGoI ,@narendramodi,@shekhar_mande ,@OfficeOf_MM
Ctd 3) Also the second variant of interest from Botswana is another one growing ,that means India should immediately stop flights from Africa and also again look at the travel hubs that have flights coming from Africa eg Dubai@HardeepSPuri,@JM_Scindia
Long thread -As a Surgeon I am painfully aware how difficult it is for a surgeon to work with a new anesthesist,or to work with new assistants/get work done in a new set up or with new nurses, there is a critical element of camaraderie, the team works like an oiled machine ctd)
Ctd2) A gentle grunt here , a small nudge there , you extend your hand and the instrument is in ur hand these arent luxuries, they put the mind of everyone at ease and causes better results in CRUX situations. HOWEVER the pandemic has disrupted these synergies@HalfwayKnown ctd)
Ctd3) Nurses are leaving , doctors & nurses are falling sick, some dying and others suffering from Long Covid. You just cannot simply replace a team member and expect them to perform, it takes time for them to settle in @DustyEnfermera,@NursesOntario ,@NursesWhoVax ,@NurseTexas
Long post ) It has been my sudden realization that even though we knew all along since last 1.5 yr plus that airplanes are the biggest source of transfer of new variants @JimRosenthal4 , @kprather88 but instead of doing more about trying to prevent in flight infection we ctd)
@JimRosenthal4@kprather88 Ctd2) we continued our apathy towards the same . Masking is important BUT as I have always maintained it has to be N95 masks in flight , we know that for every person infected there is a chance to infect occupants until 2 rows before and behind, worser this is pre Delta info ctd)
Ctd3) The same hasn't been updated , the Canadians at least did make an effort to try finding this out but haven't seen anything since last 6 months .Also we need to lay down the rules of inflight contamination levels @DrAxelrod_WHO ,@WHO ,@WHOEMRO ,@WHOSEARO ,@DrRabiWHO ctd)
Ctd2) clearly from the paper 15 kids were lost to flu when we introduced Flu vaccines in kids across UK.And how many do we have now , over a hundred have already been lost forever, a hundred families will stay up on Christmas eve and mourn, Santa will not visit those homes ctd)
Ctd3) This year flu shots have been expanded across UK . Why ?? 1) Govt wants to reduce flu levels in population BUT doesnt want to reduce Covid cases ?? 2) Vaccines against flu save kids and society both Ain't the same be said about Covid Vaccines .@TigressEllie, @Tall_Paul_05
Ctd3) This long term advocacy of the rights of children with special needs is not our hobby ,its a life long committment and passion for our team .ctd)
