Immunity is more than just neutralizing antibodies.
Immunological memory consists of antibodies, memory B-cells, memory CD8+ T-cells, and memory CD4+ T-cells. These responses are what give us enduring protection even against newly emerging SARS-CoV-2 variants.
Immunological memory is probably the most important feature of immunity as it allows us to better respond when the threat is encountered a second time. Against SARS-CoV-2, it helps to determine protection against reinfection, disease risk, and vaccine efficacy.
As long as your T-cells still recognize this virus and inform your B-cells they need to get to work and adjust to make antibodies to be better “fit” you’re protected. Imagine your B-cells are your sword that need sharpening every now and again and your T-cells are your shield.
The term infection refers to the virus entering and being detectable in your system regardless of whether OR NOT it makes you sick, whereas the term illness refers to the virus entering, being detectable in your system AND making you sick. It it important not to conflate the two.
Dr. Jansen said it was “very important” that the parts of the Omicron variant targeted by virus-killing cells were mostly unchanged from previous variants. “It gives comfort that you will have sufficient T-cell responses to prevent the worst outcomes.”
While antibodies are the first line of defense against infection, they are only part of a wider-ranging and powerful response by the immune system. Because antibodies are the fastest and easiest part to measure, those results typically come first. nytimes.com/2021/12/08/hea…
“During the first week after onset of symptoms a mild clinical course was observed. This suggests that full vaccination followed by a booster dose still provides good protection against severe COVID-19.” Keep in mind, “breakthrough” symptoms were SELF-REPORTED. SMALL sample size.
More than half the cases were reported as “mild” (think cold-like symptoms), “moderate” (think flu-like). This IS these VACCINES AT WORK. Preventing YOU from facing SEVERE disease and worse. papers.ssrn.com/sol3/papers.cf…
As MORE than 80% of epitopes in the spike protein recognized by CD8+ T-cells are not affected by the mutations in Omicron, two doses MAY STILL induce protection against severe disease.
Out of @settelab: “T-cell epitopes in natural infection? The analysis of the full proteome here show that on average 98% of CD8 and 95% of CD4 epitopes are totally conserved.
“T-cell epitopes and vaccination? Here is the analysis of spike-only data. Since many variant mutations are in spike, we expect naturally a higher number of mutated T cell epitopes. On average 94% of CD8 and 91% of CD4 epitopes are still totally conserved.”
“When we get to Omicron presumably because of the higher number of mutations, the fraction of CD8 epitopes that are 100% conserved drops significantly to 95% for CD8 and 88% for CD4 whole proteome. Still very high conservation.”
If you’re wondering what difference a booster makes, THIS is the key!
An mRNA booster on top of prior infection OR on top of two doses of vaccine confers 30X to 200X HIGHER neutralization titers versus Omicron.
Let’s talk about that! 🧵
NOTE: If you see PMS20, it is an antibody resistant spike designed for purpose of studies.
In regards to infection. Analysis of plasma from convalescent individuals (1M, 6M, and 12M after previous infection) shows substantial Omicron neutralization escape. Remember, again, we are only looking at nAbs.
According to health officials at the CDC and ECDC, the most common Omicron symptoms include cough, fatigue, and a runny nose. The data is an early sign that vaccines ARE continuing to do their job of warding off severe illness. Omicron being “MILDER” is likely DUE TO IMMUNITY.
Yesterday, the Centers for Disease Control and Prevention released a report on the first 43 Omicron cases confirmed across 22 US states. The cases were by and large mild. One person was hospitalized, discharged after two days and has already recovered. cdc.gov/mmwr/volumes/7…
Europe has identified 337 Omicron cases across 21 countries. Each of the cases has been either asymptomatic or mild, and no Omicron deaths have been reported, the European Centre for Disease Prevention and Control said on Wednesday in its latest update. ecdc.europa.eu/en/news-events…
World Health Organization: Preliminary data indicates hospitalizations in South Africa remain LOW, and emerging data indicates Omicron may cause LESS severe illness and is MILDER, but possibly more transmissible. Immunity through vaccination and previous infection is kicking in!
While researchers are still working to determine whether omicron is fueling the surge, the WHO said that emerging data from South Africa indicates the new variant may cause less severe illness. Data on COVID-19 hospitalizations across South Africa between November 14th and
December 4th show that intensive care unit occupancy was only 6.3%, which the WHO said is very low compared with the same period when the country was facing the peak linked to the Delta variant in July.
Fantastic news. Initial lab studies show a third dose of Pfizer’s COVID-19 vaccine neutralizes Omicron. A booster with the current version of the vaccine RAISES antibodies 25-FOLD providing a similar level as observed after two doses against the original virus and other variants!
As 80% of epitopes in the spike protein recognized by CD8+ T-cells are not affected by the mutations in Omicron, two doses MAY STILL induce protection against severe disease.
Blood plasma from people immunized with two doses of the vaccine had a 25-fold reduction in neutralizing antibody levels versus Omicron, when compared with the original strain of the virus, boosters RESTORE that loss.
Preliminary data on Omicron and whether it escapes vaccine antibodies shows immune escape is “robust” but NOT complete- it’s partial! Reduction in nAbs was even LESS in those who had a previous infection & two doses of Pfizer’s vaccine. Boosters should be able to take this on!🧵
VERY prelimary data so numbers subject to change but live virus studies show a 20-40 fold reduction in nAbs with two doses of Pfizer’s vaccine. ONLY TWO. This is NOT regarding boosters. CRUCIAL point: the reduction was not total. It’s partial. I know the number sounds alarming-
BUT that DOES NOT mean 40%-60% decrease in vaccine efficacy! Please don’t get confused. I told you guys these vaccines could take a hit and still hold up. Now, let’s continue.
Sotrovimab, a COVID-19 antibody treatment that has been approved in the UK, has proven to be effective against ALL new mutations in the spike protein of the Omicron variant, NOT just the key mutations. The tests included ALL 37 mutations identified to-date in the spike protein.
Sotrovimab reduced the risk of hospitalization and death in people with mild to moderate COVID-19 by 79% in trials.
Given the less than three-fold drop in neutralization during tests, developers are confident that Sotrovimab will continue to provide significant benefit for the early treatment of patients hoping to avoid the most severe consequences of COVID-19. The injected antibodies are just
