Cognitive Function in Older Adults With Cardiovascular Disease(s): @ACCinTouch and @JACCJournals

By the amazing: @NailaIjaz_MD and @cardskrish

More to come in @JACCJournals: @yasserj94 @MichaelGNanna #Aging @AmerGeriatrics

Here are few highlights: 👇👇 Image
1. 🧑‍🦳👴 The population of older adults with multiple chronic conditions and cognitive dysfunction is growing, and many will seek care in cardiovascular care (inpatient and outpatient).

We need close collaborations betwwen @AmerGeriatrics @ACCinTouch: @DrAROrkaby @minjikwak2
2. 🧠🔍 Literature shows that hypertension, metabolic syndrome, insulin resistance, and diabetes are associated with cognitive impairment/decline and share risk factors with cardiovascular disease.
3. ❓🤔 Simple questions can be asked to assess cognitive decline, such as difficulty retaining new information, repeating questions, forgetting conversations, misplacing objects, and trouble with abstract thinking.

@Yassirj94 summarized all instruments relevant to CV clinician
4. 👥📝 Asking about activities of daily living (ADLs) and instrumental activities of daily living (IADLs) can help determine functional status and a patient's ability to adhere to treatment plans.
5. 🧪👨‍⚕️ Tools such as the AD8 questionnaire and the Mini-Cog assessment can be used for initial assessment of cognitive dysfunction, and objective testing may be necessary for proper diagnosis.
6. 🩺💊 Cognitive decline is associated with poor outcomes, including frailty development, cardiovascular disease development, rehospitalization, and mortality.

On this in #JACCAdvances
7. 👨‍⚕️👩‍⚕️ Involving appropriate specialists and linking patients to resources may improve medical adherence and cardiovascular outcomes.
8. Memory, attention, and executive functioning are all necessary to adhere to medical recommendations and care for oneself.
9. Impairment in one or more of these domains can result in decreased adherence to medications, follow-up with providers, and self-care, which may explain its association with the development and progression of CVD, rehospitalization, and mortality.
10. A gauge of cognitive function and functional status is important to determine whether a patient can independently access care, remember appointments, transport oneself to appointments, and take medications.

Another reason to address polypharmacy: @ParagGoyalMD
Thank you for @ACCinTouch for increasing awareness on issues associated with #aging during cardiovascular illness.

The overarching goal is to improve cardiovascular care for older adults: @AmerGeriatrics

Angina After Percutaneous Coronary Intervention: Patient and Procedural Predictors: @CircIntv

1 in 3 patients post FFR guided PCI had symptoms, but delta FFR (pre vs post) may be useful predictor of post PCI angina!

Here is 10 point summary 👇👇👇 Image
1. The TARGET-FFR randomized trial found that one in three patients reported angina 3 months after undergoing percutaneous coronary intervention (PCI), which is a substantial proportion but reported previously in literature.
2. Defining a level of post-PCI angina that might be acceptable to patients or represent a clinically meaningful improvement would be arbitrary metric, as improved but persistent symptoms may be considered a success by some patients yet be completely unacceptable to others.

Influence of Culprit Lesion Intervention on Outcomes in Infarct-Related Cardiogenic Shock With Cardiac Arrest: @JACCJournals

Excellent: culprit-only is for AMI-shock and now for AMI-cardiac arrest - @thiele_holger

Here are few highlights, but pls read full study

👇👇 Image
1. In CULPRIT-SHOCK, a cohort with infarct-related cardiogenic shock (CS), more than 54.2% had cardiac arrest (CA) before admission.

That is quite high!
2. Patients with cardiac arrest were younger and had fewer comorbidities, but had a higher rate of impaired end-organ perfusion on admission.

Prognostic Value of Machine-Learning- Based PRAISE Score for Ischemic and Bleeding Events in Patients With Acute Coronary Syndrome Undergoing PCI: @JAHA_AHA

Editorializing: 3 major applications of AI/ML in medicine: automation, prediction, cognitive reasoning!

Summary 👇👇 Image
1/📊This study investigates PRAISE score for predicting 1-year all-cause mortality, recurrent AMI, and major bleeding events in post-PCI ACS patients treated with DAPT.

Application for #prediction in medicine using 6433 ACS pts
2/🔍Study finds PRAISE score has moderate discrimination for all-cause mortality but poor discrimination for recurrent AMI and major bleeding in an Asian population.

Critical Care Enrichment During Advanced Heart Failure Training: @JACCJournals

By the great: @JasonKatzMD and team.

- The title should have read advanced heart failure and interventional cardiology training!

Highlights below.

👇👇👇 Image
🚨 Critical care cardiology (CCC) is a rapidly growing cardiovascular subspecialty.
💡 Critical Care Cardiology is attractive to trainees because of high patient acuity, complex decision-making, and focus on multidisciplinary care and research opportunities.

- Remember the cath lab is a mini-CICU

Association of Incident Cardiovascular Disease With Time Course and Cumulative Exposure to Multiple Risk Factors: @JACCJournals

Nice analysis from Cardia

Here are few summary points 👇👇
1️⃣ This study quantifies CVD risk after age based on cumulative exposure to multiple independent risk factors.
2️⃣ Model takes into account time-varying severity of risk factors.

Right Ventricular Failure: @NEJM

In-depth review on the right heart: @Brian_Houston12 @RyanTedfordMD

Will try to hit the highlights, but cannot substitute for the details in the full text!

👇👇
1️⃣ The right ventricle of the heart plays a crucial role in various conditions, including left heart failure, pulmonary arterial hypertension, and even COVID-19 infection. 💔🩸🦠
2️⃣ In 1943, Isaac Starr and colleagues performed animal experiments that concluded "weakness of the right side of the heart seems less important" in heart failure dynamics. 🐭🧪

(Great attempt, but clearly RH plays a critical role)
