Denied freedom.. a 🧵

Denied freedom to

shop for a prom dress

walk the mall with your child

go to your doctor

worship

go to the supermarket

go to school

teach in school

To do these things without being snuffed out, there one second and gone the next

Freedom to breathe, to come home that day

Freedom to access life, liberty and the pursuit of happiness ->

But freedom is layered, complex; each of us sees &lives it differently, we prioritize freedoms individually

Who’d have thought freedom, as foundational as we purport it to be, is so individually interpreted and variegate?

Why should a doc be tweeting this? ->

.. bc it’s in the societal grey of freedom, with its different interpretations, where some of our patients die, &die violently &early, so we’ve got to ask/understand/speak/advocate

Some other freedoms that are far reaching:

Freedom to bear arms
Freedom to bear any arms

Freedom to carry arms
Freedom to access guns
Freedom to love freely
Freedom to deal with failed contraception as one sees fit
Freedom to live through an ectopic pregnancy or survive a pregnancy threatened by sepsis
Freedom to treat a patient in medical trouble

Freedom to see a doctor for mental health
Freedom to vote
Freedom to peacefully protest
Freedom to gerrymander
Freedom to impinge one set of values or worldview on another

Freedom to avoid being snuffed out by someone else’s lens on freedom

Well, freedom isn’t so straightforward ..

Some of the above listed freedoms may be seen as inviolate by some, not so by others. Some may seem wrongheaded to some, but not to all

As physicians/nurses/respiratory therapists/chaplains &teams, though, WE -> ..

… see life leave via the dangerous mess that is the societal grey of freedom

Our disparate freedom views can snuff out the seven, sixteen, and twenty three year olds with bullets, and can snuff out the expectant Moms with complications

Encumberability - it may not be a real word, but some of the freedoms above should be unencumbered - all those that make life, liberty and a pursuit of happiness fairly tangible

Some of the freedoms above, however, need to be responsibly regulated ->

there needs to be clear acknowledgment - freedoms that can curtail, menace, threaten life and peaceful agency of another, .. those freedoms need to be encumbered, and need to be responsibly accessed and applied w the greatest of respect and common sense

These are our lanes. Many of our patient’s very lives are at stake, as is a reasonable expectation that we all continue breathing through &survive a random day

These metrics are from CMS menus, or from finance, etc

EMRs don’t always generate panel wide data well, don’t always define or find panels well. #primarycare metric measurement workarounds then have to be created repeatedly

Primary care docs fulfill an indispensable role not just in the local setting, but we provide what all the country and our society desperately needs - coordination and high value care in a system that overspends and doesn’t provide universal access nor competitive outcomes

My Aunt Joanie loves her classical music, I got it from her

Yesterday on road 2wedding (see nice posts) played mellow rock 4the car, not like classical for her

I caught her index finger tapping to the beat of a few, sharing->

Then a voice came up that I will confess as a teenager I crushed on. Aunt Joanie tapped slowly: what an interesting voice, singing about Sara

1/ Debating whether to dignify this ... but here goes

I think most of us endangered docs/nurses/respiratory therapists/social workers/hospital chaplains are also frustrated re clarity of the campaign &publicly voiced narrative re this administration’s &our nation’s response ->

The ‘it didn’t have to be this way’ - must remind the public, the on the fence voter, of the following:

Pandemic preparedness structures were pared down &not present 2help initial strategy

‘Leadership’ must b holistic/humble - it sits back &listens deeply to expertise ->
3/ if ‘leadership’ respected lifelong career pandemic preparedness experts, we would have:

accepted WHO tests for #COVID19 to apply at the outset

commissioned efforts 2create a new test 2multiple entities 4redundancy as an insurance policy, ie .... ->

1/ This past Thurs, I was honored to ‘emcee’ a grand rounds @HofNorthwellDOM, presenting the Candee Award for Education Excellence in Medicine 2our beloved Ron Rosen of @NorthwellDGIM

Ron is a 50 yr general internist who has taught no less than 40 yrs worth of learners -> 🧵
2/ Ron had so much to tell us about his aunt and uncle, who were early family medicine practitioners in the Bronx/Manhattan. Ambulance drivers would take them to a patient’s home. A beautiful sepia tone showing his aunt going to a call, and another of his uncle in the office
3/ We heard about Dr. Rosen’s medical school yrs @nymedcollege in the early 60s &how he knew the civil rights movement, the movement for reproductive rights, the attempts to mitigate poverty and the birth of Medicaid and Medicare would affect all his future pts &his work ->
