Acne can be one of the most rewarding yet frustrating conditions to treat. Rewarding because some people respond to treatment so rapidly, frustrating because acne is manageable rather than curable and relapses occur often.
2. It is so common that it is impossible not to have patients at my clinic with acne in a week. Adolescent acne tends to be worse in boys while adult acne(acne in people 25 years and above) is worse in females.
Did you know that for adolescent acne, even without doing anything,
3. it resolves in a lot of cases on its own? Especially when you don't pick your face and you don't do all the wrong things....like putting lemon on your face?

That combination you mentioned up there has lemon which can cause irritant dermatitis for a lot of people.

I was tagged to a post on Facebook of someone that had used an organic product gone wrong. If it wasn't a colleague that tagged me, I would have just ignored. Because it seems the voices of caution and reasoning are no longer listened to. Image
2. People have adopted the "if I perish, I perish" attitude.

Any product that claims to be organic and turns you five shades lighter in two weeks is nothing but trouble.

Organic but gives you patches of colors is nothing but bad news.

It is just sad that people that have good products with good quality control are affected because of all the rubbish people are parading out there.

It is VERY easy to damage the skin, but repairing may become a herculean task.

Let's talk moisturizers
Moisturizers refer to compounds that moisten (as the name implies) and hydrates the skin thereby protecting it from dryness and helping to maintain the integrity of the skin. They are useful in both normal skin and in skin diseases

Discoid eczema. Cause not exactly known but may be associated with dry skin (xerosis), contact dermatitis, some drugs, nutritional deficiencies amongst others. May run a relapsing/remitting course.
Discoid means coin shaped because of how it looks like
It usually occurs more in the extremities especially upper ones. Eczema in dermatology especially acute one refers to a skin with all the features of inflammation; redness, itching, vesiculation and exudation of fluid. Pityriasis versicolor aka 'ifo' is NOT ezcema please
This may also for discoid eczema. Most people tend to say tinea corporis, but remember straightforward tinea corporis has the centre clear with scaly raised edge. Signs of inflammation not common except it the zoophilic forms.

Yesterday was World vitiligo day. A condition with an unknown cause however largely attributed to autoimmune destruction of the melanocytes leading to absence of pigment in the affected areas.
If you are asked for differentials...please do not let albinism feature in that list. It is NOT usually a differential of vitiligo. Except maybe in vitiligo universalis where almost the whole skin is affected.
Albinism refers to a group of inherited disorders where there is a problem with melanin synthesis due to the deficiency of some enzymes or proteins in the melanin production pathway.The melanocytes are fine, it is the melanin production that is absent. It doesn't occur in patches

1. This post was inspired by a patient that I saw recently(and I told her I was going to do this). Let's talk about 'facials'. A popular procedure done by spas. You're unlikely going to find a proper aesthetician or dermatologist talk about that.
2. What you'll get is they'll tell you the EXACT procedure they need to do.
When the word "facials" is used...it can mean a wide variety of procedures from an extraction of whiteheads to a hydra facial, to a microdermabrasion, microneedling, antiging facials where some serums
3. Are used after deep cleansing the skin. Note that any procedure done on the face is aimed at reducing signs of ageing and sun damage and ultimately rejuvenation.

This patient that inspired this post had a clear face before going for the said facials.

A few tips for my colleagues when it comes to the skin
1) If the skin lesions come up rapidly with constitutional symptoms like catarrh and fever especially in children, it's most likely a viral exanthem. Treat symptomatically. Don't be too quick to prescribe antibiotics.
2. If you find lesions on the skin in the neonatal period, and the child is not irritable, no fever or other constitutional symptoms and the child is feeding well...most likely one of those transient neonatal lesions due to maternal hormones. Reassure the parents alone.
3. Read up Pityriasis rosea. There would come a time during the year where there'll be clusters of it. Don't be in a hurry to prescribe antibiotics; might make it worsr. It is usually self limiting. I have done a thread about it before. Use the #dermatologyteaching to look for it

Its commonly called 'malaria blister', 'fever blister' or 'isirun' in Yoruba language. Most people would have had it at one time or the other. I'm talking about Cold sores or herpetic sores. Caused by a virus called Herpes simplex virus.
There are two types of this virus. Herpes simplex virus (HSV) 1 and 2.
Type 1 causes 85% of oral sores while type 2 causes 85% of genital ulcers. It is important to note that either 1 or 2 can cause infections in oral or genital areas. Anyone exposed to the virus can get infected
Especially in cases where the immune system is 'tired'. That's why its not unusual to find it in people with febrile illnesses.

Let's talk about a condition called papular urticaria.
This common condition is often seen in infants and children.
Usually due to a hypersensitivity to bites from insects, fleas,mosquitoes, mites, bugs...etc. The group of people that develop papular urticaria are thought to develop a blood spread material (an antigen) after the bite which then causes them to have a generalized reaction.
Although the reaction is more common on the limbs, it also can be seen on the trunk and other areas. So in essence, after a bite.. people with papular urticaria will have more generalized itching and swelling apart from the bite area.

This particular thread is a reminder. My colleague @flakydancindoc and I were talking yesterday and we agreed to both talk about this as it's like there's an outbreak. It's also one of the neglected tropical diseases. SCABIES again.!
This condition is caused by a mite (like an insect) called sarcoptes scaibei. It is presently being misdiagnosed out there a lot. AMD the diagnosis is often missed causing distress to the patients. There are some reasons why the diagnosis is often missed.
1) Some health workers think that it affects people in the lower class only which is completely false. Infact....I'm seeing it more in my Lekki and Ikoyi people now😁. All you need is close contact to someone that has it. And the infection can be propagated far and wide

To use or not to use?
Let's start from the basics.
Almost every part of the body except for some sites we call sanctuary sites have something we call normal flora.
These are microorganisms that are put there by nature. Some areas have way more than others e.g the mouth..
These organisms are called normal commensals. The two of you are living in peace with each other. You're the landlord and they're tenants
They pay rent by living there and preventing the growth of harmful or pathogenic organisms.
So that staphylococcus they're always telling you is on your skin..is a normal flora. I usually laugh in Swahili when they alarm people about Staph on the skin😂

Let's talk about triple action creams. Infact I saw a quadruple action cream yesterday and I was like😮😮😮.
Aka gbogbonìse or one size fits all as I like to call them. It is important to talk about it because they have become like oxygen. You'll find them EVERYWHERE you go.

What do triple action creams contain?
Usually a combination of a high or mid potency steroid like clobetasol dipropionate or betamethasone dipropionate, an antifungal like clotrimazole or ketoconazole and an antibacterial like neomycin.

Steroids are drugs that are used to calm inflammation.

It's been some time hasn't it?
I'm choosing a topic that I have gotten up to three people ask me about it in the last one week.
Do you scratch your body after bathing? You may have what is called 'Bath related pruritus'. Pruritus just refers to itching😊
So some people for one reason or the other start to scratch like someone that has 'werepe' on their skin after bathing. Intense itching that happens about 5 minutes after bathing. Several causes have been ascribed to this. I'll talk about the common ones.
1) Aquagenic pruritus which is related to the water used in bathing. Some people scratch after they come in contact with the water. There are some itch mediators that are released causing the person to scratch. More common with hot/warm water but note that it can occur with any

Have you ever heard of the condition called herpes zoster?
This condition is caused by a virus called Varicella zoster virus. It is infact a reactivation of the virus in the dorsal root ganglion (within some nerves) after it finds a way to hide there after an initial chicken pox infection probably decades earlier.
This means that after a person has had chicken pox, your virus hides in that place in the body and becomes reactivated later. This reactivation does not occur in everyone. Often occurs due to immune suppression of some sort.

Have you ever seen these before?

Of course you have. It's so common in blacks that as you're reading this you might find one or two on your face or neck.
Dermatosis Papulosa nigra (DPN) and Seborrheic keratosis
These are just benign harmless lesions that are more of a cosmetic embarrassment than anything else. It has been noted to have a familial tendency so it is not uncommon to find it clustered in members if the same family.
DPNs are believed to be a subtype of Seborrheic keratosis which tends to appear flatter. Commonly affected sites include the face, neck, trunk and arms. Usually increases in number with age. May become more extensive with pregnancy.
The cause is not known.

She was the life of the party. Beauty and brains combined. Beautiful in soul and looks. At 19, she had already completed her first degree at the prestigious 'better by far' university in Nigeria. Her parents had just two of them. And they were as different as night and day.
While Bimpe was kind and loving, Bayo was cruel at heart. While her parents could go to sleep with their two eyes closed when Bimpe was out, it was always fasting and prayers when Bayo was around as he always had calamity following him.
