#idtwitter 4 friends travel from NYC to Florida: surfboard during presence of red tide. +sore eyes & wheezing on beach. They ate steamed crabs 3d prior and ate Chinese food <24 hrs prior to presenting with respiratory sx which lasted 48 hours. Diagnosis? #medEd #idmedEd 
aerosolized #dinoglagellate toxin due to inhalation of dinoflagellate toxin #brevetoxin related to #redtide. Generally inhaled via wave action during #algalbloom where Karenia brevis toxin is inhaled and exposed to eyes
Illness in mild & short. Steamed crabs are low risk inthis case given consumed 3d earlier. DDx: ciguatera produces neurologic features, IC 30min-36hrs.More severe & pulmonary sx are atypical. Chinese restaurant: MSG could cause bronchospasm IC 5-30min though
Ddx: scombroid can cause ocular & respiratory sx but last only few hours and shouldnt worsen respiratory function. Ddx: vibrio parahemolytics will cause diarrhea
