1/ Booster Doses - 9/12 later

Worth it? Rationale?
With global vaccine inequity - should we feel guilty?
Why I chose to mix...

Also co-incidentally this is my Halloween costume:

"Government Agent"
"Big Pharma Shill"

Take your pick.

Some thoughts ahead.

2/ Why Now?

First dose Dec 20 & second dose Jan 31st.
At the time we weren't sure what optimal dosing interval was & had ongoing supply issues, so many got 2 doses at varying shorter intervals.

Now we know ~12 weeks is a good interval, but also are seeing waning immunity.
3/ Waning Immunity

Data is showing our antibody levels to #COVID19 are decreasing, but that doesn't mean our overall immunity is, as we are seeing by our vaccine effectiveness data in BC.

But, in BC we still are losing too many ppl, almost 50 have passed in last week.
4/ What data do we have on boosters?

More to come for sure, but Pfizer released data last week, RCT of 10,000 ppl that showed post booster

"relative vaccine efficacy of 95.6% against disease during a period when Delta was the prevalent strain"

pfizer.com/news/press-rel…
5/ How long will boosters last?

Don't know yet, but based on vaccine science we can expect that a booster like this may induce longer term immunity as we have seen in Hepatitis B, Diphtheria, Tetanus, Pertussis, Polio, & Haemophilus influenzae type b Vaccines etc.
6/ How can we in good conscience get our booster when many in the world haven't gotten first?

We know the world is suffering, but as HCW we have a responsibility to make sure we're keeping our patients safe, so as our authorities recommend it, we will get it to do our part.
7/ Canada - COVAX contribution

Canada made more commitments at G20 today.

"In all, between financial contributions and direct delivery of doses, Canada says it will help deliver at least 200 million shots of vaccine by the end of next year."

globalnews.ca/news/8337703/c…
8/ WHO Statement on Boosters

"Introducing booster doses should be firmly evidence-driven and targeted to the population groups in greatest need. The rationale for implementing booster doses should be guided by evidence on waning vaccine effectiveness..."

who.int/news/item/04-1…
9/ BC Boosters

Our program is proactive & most ppl will be getting boosted in 2022 (6-8 months post 2nd dose).

The boosters now will be targeted for people who there is higher risk of waning immunity or are in higher risk situations.

I believe the spirit is concordant.
10/ Why Moderna?

I got Pfizer for my first 2 doses, and decided to Modernaize my approach (was given the option w/ thanks).

Some data is emerging showing mixing booster may boost antibody response w/ similar side effect profile & no safety concerns.

11/ More on Waning/Boosters

Read this eloquent piece by @jodyvance (written last week)

"Now here’s my well-protected, double dosed self, whining about waning immunity? This is the COVID-19 equivalent of a first world problem."

Food for thought.

12/ Global Perspective - Israel

Reminder at the end of summer, well into their double-vax program,

"have been doubling every week to 10 days since then, with the Delta variant responsible for most of them"

They implemented boosters.

science.org/content/articl…
13/ Israel Now

"As the booster campaign ramped up, new Delta infections, hospitalisations and deaths began to dramatically tumble. More than a third of the Israeli population has now been triple-vaccinated."

95-99% cases in hospital are unvaxxed.

abc.net.au/news/2021-10-3…
14/ Boosters

So, should you get boosted?

As always, depends on community & personal risk, time since 2nd vax, work/life situation etc

Whatever you do, humbly remember, we are fortunate to live in a society where we have access to these life-saving vaccines.

1/ #COVID19BC - Vaccines in 5-11 year old - Thoughts

This morning it was announced (by media) that 5-11 yo can now be registered.

A friend asked me if I had a child in that age range, would I be getting them vaccinated?

I said probably, but that isn't good enough...

2/ #COVID19BC - Questions

Before we get into whether it makes sense yet, why do we keep hearing this stuff from anywhere but Public Health?

Another "soft launch" of systems? Like vax bookings, card registration & now this kid vax reg system, @BCGovNews announces after launch.
3/ #COVID19BC - Questions

This may be done intentionally, but why do it on a long weekend, when people aren't available to answer questions about it officially?

Communication from PH has been my biggest issue throughout the entire pandemic & it seems to continue.

1/ Vaccines in Canada:

We all heard that #AstraZenecaVaccine was approved for use in Canada on Friday.

We now have 3 #COVID19 vaccines, but with that there are discussions re: "Which one is the best vaccine for me?"

Docs say "the best vaccine, is the one in your arm".

Why?
2/ Some thoughts:

But AstraZeneca is only ~62% effective against COVID? That's not good enough, right?

We need to examine & reflect on where we were, and where we are going.

March 11, 2020 - @WHO declares global pandemic.

Within 1 year, we have 3 vaccines (in Canada).

WOW!
3/

1 year ago, we would NOT have thought we would be in this place now.

It's a testament to a concerted global commitment to scientific excellence, that we have these available to us now.

It has definitely been a struggle, but we are making tremendous progress.
