For a long time a doctor who refers to a book / internet in clinic has been looked down upon by Indian public.
Such doctors are seen as not intelligent or poorly trained and even inexperienced. #thread
Millennia old tradition of treating memory as THE indication of intelligence is so ingrained in Indian mind that most people will not even consider looking at the calendar to realise that we are in twenty first century.
So back to clinical practice -
Prescription medicines interact with each other and many of these interactions are not safe for the patient.

So reviewing a prescription with multiple drugs is a doctor's nightmare.

When patient is a 8 yr old child, it is like 🧟‍♂️ apocalypse.
For years desktop reference books have helped doctors in this task to keep patients safe from harmful drug interactions.

Though Indian public love to see a book cupboard filled with fat medical books and journals in doctor's clinic, they don't take kindly to a doctor
who actually reaches out to a book right in front of them.

I have heard nasty feedback about this habit of mine since the day I started private practice in 2000.
Now internet based interaction checkers are a blessing and I use them frequently.
So yesterday when I was faced with a long prescription of a child with multiple physical illnesses and now in need of anti depressant medication, I told them - hold on. This is going to take a while.
And to my utter surprise, parents of the child flashed a reassuring smile.
At the end of the session, patient's mother remarked - glad you checked it. Now we feel safe.

Lady, may your tribe grow!

Is my india really changing for better? :)

We all are exposed to huge amount of negative news on daily basis.
It percolates inside us gradually, unknowingly and affects how we feel about ourselves and our society.

Here is a short thread about this negative onslaught and protecting our mind.
You may not be consciously aware but constant negativity about education, healthcare, society, economy, etc. primes you with negative expectations and makes you notice even tiniest negative aspects of everyday life.
This bias towards negativity leads to stress and more negative emotions on a daily basis.

Even casual conversations steer towards such topics and stay there.

I often hear parents saying - we have told him/her that if you are not great in studies, it is okay. We can make you a sportsman/businessman!

My heart sinks when I hear this.
Let's talk about the "sportsman" part of this.

As a child, if you want to take up any sports as a serious persuit in life, you aim for the international stage.
Being in top 5/10 in the world, Olympic medal, representing India at international level, etc.

This would be a reasonable goal for a child (less than 10yr).
Unless you have that kind of ambition / desire / 🔥 in you, professional sports is not really your thing.

#thread #secondopinion

I am all for getting a second opinion.
It is extremely useful in - serious diagnosis, serious and expensive treatments, need of irreversible treatment like removal of parts/organs.

Some suggestions to make your spend on second opinion productive -
1. Get a written diagnosis and treatment plan from your first doctor and tell them clearly that you are going to get second opinion. Clearly state your reason. No good doctor will be offended by this line of action.
2. Choosing second opinion doctor - ask friends, relatives or trusted family physician. Past experience of others is usually a reliable indicator.
Important - web pages, media presence, ratings and awards are negative indicators.
Almost ALL awards are available for sale.

Real life child psychiatry. #Thread

22 year old lady is grinning from ear to ear as she gives me a box of sweets to celebrate her success. She finished her graduation and is planning for MBA now.
Her mother is standing there with tears of joy running down her cheeks. Only if she could speak freely....
Being a deserted woman with two children is a difficult life even for better off. For a woman with basic degree and basic job, it is almost impossible task.

Govt has most mysterious ways of working.

Punish for use of Bluetooth devices while driving but don't ban sell of those devices.

Punish doctors for listing on aggregator sites but don't ban these sites and their business model!
How does this even work?
Penalise doctors for writing branded medicines on prescription, then why are these medicines allowed to be manufactured and marketed?
Punish doctors for taking hospitality from Pharma but allow Pharma to take tax breaks for this spending.
