Women have a higher risk of heart attack, heart failure and stroke at a lower BP as compared to men

1. The myocardial infarction (MI) risk for women with systolic BP (SBP) 110 to 119 mm Hg was comparable with the MI risk for men with SBP ≥160 mm Hg.
#MedTwitter #neurotwitter
2. The heart failure (HF) risk for women with SBP 110 to 119 mm Hg was comparable with HF risk for men with SBP 120 to 129 mmHg.
3. Stroke risk also manifested at sex-specific, albeit higher, thresholds:
Stroke risk in women with SBP 120 to 129 mm Hg was comparable with risk in men with SBP 140 to 149 mm Hg.
doi.org/10.1161/CIRCUL…
4. Normal BP range among women and men differ.
The maximum sex difference was found at age 26 (+8.2 mm Hg higher in men); women then experienced steeper rises and caught up with men by the seventh decade.
doi.org/10.1371%2Fjour…
5. What are the implications of these observations?

*Research is skewed towards data obtained from men, and the representation of women in research is lower (or nil in many cases),
*The normal SBP range is lower in adult women (as compared to men).
#gendergap #research
6. Take home message
*Systolic BP targets should be lower in women (as compared to men) in order to reduce the risk of heart attack, heart failure and stroke.
*Aim for SBP<100 mmHg in women, and <130 mmHg among men.
6a.
*SBP 100 to 109 mm Hg relative to SBP <100 mm Hg was associated with incident Cardiovascular disease in women but not men, in whom risk was seen at SBP 130 to 139 mm Hg.

1. Abhi got late at office and reached home only by 11 PM. He opened the main door using duplicate keys, so that his wife Priti won't be disturbed.
But he was shocked on entering the bedroom. He asked the woman-"who are you & what are you doing in our bedroom?"

2. Now, it was Priti's turn to get shocked- "Abhi, you came home late on the pretext of excess office work, but looks like you had a party with friends. How much alcohol did you have?"
Abhi found the voice & tone familiar; the dress woman was wearing was also familiar to him.
3. Abhi could recognize Priti now, with the help of her voice and dress she was wearing. He also noticed the familiar wedding ring she was wearing.
He could somehow manage to pacify Priti. Priti assumed that Abhi was playing some kind of prank and they both retired to bed.

1990: I was hospitalized with severe upper & lower back pain. Pain was so severe that I was bed-bound. It used to take 30 min to get down from bed & enter the washroom (no family member could attend on me).
I had just completed my first yr MBBS.

2. Doctors had started 4 different types of pain-killers, which were still ineffective in relieving my pain. To make matters worse, no definite diagnosis had been made. A senior Orthopedic surgeon was consulted to explore the option of giving steroid injection in affected joints.
3. He advised to wait for another two days for pain-relief to begin (15 days had passed without pain relief). Failing that, he would proceed with injections. As per his expectations, pain relief started on day 17.
In addition, HLA B27 returned positive confirming the diagnosis.

Data was collected from 8601 unique research papers published earlier on this topic. doi.org/10.1136/bmj-20…
2. Highest versus lowest dietary sugar consumption was associated with increased body #weight (sugar sweetened beverages) & fat accumulation in liver.
*Each serving/week increment of sugar sweetened beverage consumption was associated with a 4% higher risk of gout.
#MedTwitter
3. Each 250 mL/day increment of sugar sweetened beverage consumption was associated with a 17% higher risk of coronary heart disease & 4% higher risk of all cause death.

Lack of sleep as well as excess sleep are associated with increased risk of stroke

1. A recent research looked at the association between sleep disorders (in the previous month) and risk of stroke in 4496 people.
n.neurology.org/content/early/…
#MedTwitter #neurotwitter #sleep #stroke
2. Short sleep (<5 hours), long sleep (>9 hours), impaired sleep quality, difficulty getting to sleep or maintaining sleep, unplanned napping, prolonged napping (>1 hour), snoring & breathing cessation (OSA) were all significantly associated with increased odds of acute stroke.
3. Short sleep (<5 h) was associated with a 3.1 times higher risk of stroke, and long sleep (>9 h) was associated with 2.7 times higher risk of stroke, as compared to those who slept 7 hours.
More the number of sleep disturbances, greater was the risk of stroke.
