While the 4th wave had been expected for months and was in development in limited areas (Khuzestan) within Iran, now according to official reports it has spread all across the country.
The B117 strain (a.k.a British) seem to be the main driver.
[Current Outlook/2]
It has been very surprising that the 4th wave did take quite a while to develop which is probably due to development of partial herd immunity. And it is likely that the current resurgence is direct result of the new strain (B117).

As for the official numbers all these show confirmed 4th wave:
OVERALL, outlook is dismal since there is no prospect for vaccination.

I received the 1st does of the AstraZeneca vaccine. Had some pretty severe reactions such as crushing body ache, headache, unstoppable shakes and malaise. I was OK 24 hours later and fully recovered 72 hours after.

My brother in Iran contracted COVID for the 2nd time, confirmed by the test. He had really tough and prolonged symptoms for two weeks. His O2 level dropped to 92.
He has recovered since and feels better. This came a year after his 1st episode during the 1st wave.
The vaccination in Iran has been a complete sham. While the medical staff were defined as the 1st priority group, in lack of any official numbers, anecdotal reports among my friends show 10-15% vaccination while the number of doses imported was enough for the whole group.
It appears that the imported dose have all used up and there is no prospect of importing any more.
The official word is that widespread vaccination will not start until October 2021!
Now it is clear (if there was any doubt) that the sanctions had zero impact in ability to purchase vaccines. It is all due to lack of "will" preferring to spend the funds differently [missile program?]
There are a few dozen programmes for making vaccine - so far nothing.
Bloomberg pointed out that in a number of countries including Iran, with this rate of vaccination it would take 10 years for the whole population to get vaccinated.
The government, in its ability (or lack of will) to provide vaccine has allowed privately owned companies to import COVID vaccine. [Such companies have later said they are unable to acquire vaccine]
Look guys - THE SITUATION IN THE MIDDLE EAST IS PRETTY SERIOUS.
(I know you might say when it wasn't but we are talking risk of a big conflict)
The thread on the reports from the last 24 hours: /0
On the Iranian side, I have seen anecdotal comments from tweeps close to IRGC that "the big fight" is very close and mothers should prepare for the martyrdom of their sons.

Israeli Air Force commander describing the situation in the middle east as "explosive".
And this one from the commander of the Iranian Air Force: "Any [foolish] adventurous action in the region by the adversaries will be responded with a strong response".
