SF declares #monkeypox a state of emergency. SF has a large & politically powerful LGBT community & one of the largest MPX outbreaks in the US on a per capita basis. 1/
SF was, I believe, 1st US city to issue an emergency & take steps like a shelter-in-place order for COVID. Not sure new emergency means in practice. What we need is more vaccines, but that’s up to the federal govt. 2/
I’m quite sure @SF_DPH (run by a gay man) won’t take steps the LGBT community opposes. I don’t see a raging debate within that community, or btwn that community & officials, like we did when the city shut down bathhouses to curb AIDS in the early ‘80s.

.@CDCgov's latest COCA update on #monkeypox epidemiology, testing, vaccination & treatment. Now 3591 cases in US. 99% male at birth (possibly some trans women), 13 people female at birth (includes some trans men). All but 5 cases attributed to sex between men. 1/
These COCA updates provide a wealth of info. This one includes testing update (now capacity of 80K tests per week, but still only lesion swabs), revised simpler protocol for #TPOXX treatment. 2/

I've seen some comments on trans reporting. It doesn't say so on this slide, but @CDCgov previously reported there were no trans women & "some" trans men among monkeypox cases to date. @SF_DPH, which is more clued in on this, reports the same.

You’re not the only one frustrated @kakape. Health officials & advocates emphasized the universal message so relentlessly that it’s very hard to shift. Anyone can get #monkeypox — but everyone isn’t getting monkeypox.
I feared back in May that this messaging strategy — entirely well-intended to reduce stigma — could backfire. It seems like the people most resistant to shifting it are straight women. 2/
Listen to what gay, bi & trans men are saying (I know it’s not unanimous). Focusing resources where #monkeypox is spreading now is the best way to both reduce suffering & protect other groups. 3/ slate.com/technology/202…

The message that anyone can get #monkeypox isn't incorrect, but it is misleading. Fomites like bedsheets don’t carry the same risk as sexual or other close skin-to-skin contact. And unclear messaging is causing unnecessary panic. My latest for @Slate. 1/

You’ve all seen the warnings. You might get #monkeypox from trying on clothes. The virus lives on surfaces for months -- & it might even be airborne! Maybe you should wear a full PPE suit. 2/

But in this outbreak, #monkeypox is mostly spreading during sex between men (95% in a recent study) & like most STIs, it can be transmitted other ways as well. 3/

Here's my @pozmagazine report on the @NEJM #monkeypox study. Other reports have covered the key findings, but I'll note for the #HIV audience that 41% of cases were HIV+. The good news is that they did not have worse outcomes.

"Whilst people with HIV account for more than 40% of cases so far, it is reassuring that HIV status was not linked with monkeypox severity,” said @drlaurajwaters, chair of the @BritishHIVAssoc.
The proportion of HIV+ people in this case series appears higher than HIV rates among men who have sex with men overall. It's not clear if HIV might be associated with greater monkeypox susceptibility, though it seems possible that immunosuppression could play a role.
