Thread on #Favipiravir and the data on which approvals are being granted to it across the world. First used in Japan, then China, and in other countries: all on “compassionate grounds”. But does it really work? Read this paper. kansensho.or.jp/uploads/files/… details follow. 1/n
Favipiravir study in China: “primary endpoint of better clinical improvement at 7 days was not met” but still goes on to claim that fever and cough were reduced. So why that doesn’t count as clinical improvement?? Or was it just subjective opinion unsupported by data?
These are the details of improved, unchanged or worsened cases from the study, which suffers from extremely serious flaws. No control and test group for comparison, no comparison with known rates at which improvement or deterioration occurs without medicines.

There's buzz about #Glenmark new drug #Favipiravir for #COVID__19
Do we have the data based on which approval was granted? @CDSCO_INDIA_INF
Given the desperate situation, physicians & patients are likely to flock to get this, but do we have some hard outcomes to show it helps?
I tried to find published studies on #Favipiravir but haven't yet found supportive data in the searches I did. Would be grateful if someone could point out the evidence. And I hope it's not a single arm study where x% improved because x% could have improved anyway.
If our standards for other drugs are well conducted randomized trials, let's not lower the bar for #Favipiravir just because someone's marketing it, and we don't have any eligible candidates at hand. #EvidenceBasedMedicine

🚨BREAKING: preliminary data from an ongoing clinical study of #remdesivir showed that in 113 patients with severe #COVID19:

▶️Fever/symptoms rapidly improved
▶️Most discharged within 6 days
▶️2 died (1.8%), much less than expected for severe #COVID19

statnews.com/2020/04/16/ear…
#Remdesivir is being studied in 2 clinical trials:

1⃣severe #COVID19 study of 2,400 patients from 152 sites all over the world
2⃣moderate #COVID19 study of 1,600 patients in 169 sites.

Results for the first 400 patients in the "severe" study are expected later this month.😃
I spent nearly a decade developing the closely related #galidesivir (BCX4430, we published in @nature) that is also being tested in #COVID19 patients.

These drugs decrease viral replication by blocking the virus' ability to make new RNA.

🚨BREAKING: #remdesivir shows encouraging preliminary results for #COVID19:

▶️68% (36 of 53 patients) had improvement in oxygen support; only 15% worsened
▶️57% (17 of 30) on ventilators taken OFF
▶️3 of 4 on ECMO (*extremely* ill) taken OFF

washingtonpost.com/business/2020/…
No new safety signals, but 13% of these severely ill #COVID19 patients died.

These preliminary results from this compassionate-use study need to be confirmed in placebo-controlled trials –but these are already underway!😃

Another antiviral drug with the same mechanism of action (#galidesivir) is also being studied in a well designed, randomized, placebo-controlled clinical trial.

The scientists are ON it.😃

⚠️Good news! BioCryst launched a clinical trial of its antiviral #galidesivir in patients with #COVID19.

Galidesivir has broad-spectrum activity in vitro against >20 RNA viruses, including #Ebola, MERS and #SARS (closely related to this #coronavirus).😃

finance.yahoo.com/news/biocryst-…
#Galidesivir works similarly to #favipiravir (approved in #Japan/showed efficacy against #COVID19) & #remdesivir.

The drugs block the virus’ ability to make RNA copies needed for new viral particles, so it’s plausible they'll work against #coronavirus.🤞🏼
I spent years working on #galidesivir (BCX4430); we published our findings in @nature.

Like #SARS, #Ebola & influenza, the new #coronavirus uses RNA as its genetic code. Blocking the ability to copy RNA is an effective approach against other RNA viruses😃

⚠️Great news! @GileadSciences is donating 1.5 million doses of its experimental antiviral drug #remdesivir that could treat 140,000 #COVID19 patients. The drug will be FREE for "compassionate use, expanded access and clinical trials.”

#Remdesivir inhibits viral replication by blocking the ability of the virus to make new copies of its RNA genetic code.

I spent nearly a decade developing a similar drug (galidesivir/BCX4430) to fight #Ebola & other deadly viruses like this #coronavirus.

Another drug in this same class (RNA-dependent polymerase inhibitor) is #favipiravir, approved in #Japan for the treatment of #influenza (another RNA virus).

Favipiravir reportedly was effective in treating #COVID19, so #remdesivir may also work.😃

THREAD (1/X) It's interesting that favipiravir (Avigan), is getting relatively little press in U.S. and western countries compared to #hydroxychloroquine (HQ). #COVID19 pilot data at least as promising as any currently available HQ data presented thus far.
3/X Currently data supporting FPV (oral) comes from a Chinese open label pilot study.
It was NOT randomized, but patients were consecutively screened, which means they weren't cherry picked. Lopinavir/ritonavir (LPVr) was used as an active control
sciencedirect.com/science/articl…
4/X The LPVr arm was enrolled THE WEEK prior from consecutive screened pts. LPVr was not shown to have benefit in COVID19 pts with severe disease, a population excluded in this FPV trial nejm.org/doi/pdf/10.105…

🚨BREAKING: #China says a flu drug approved in #Japan was effective in a study of 340 #coronavirus patients.

Lung X-rays showed improvements in ~91% of patients treated with #favipiravir, compared to 62% of those not treated with drug.

@thespybrief
theguardian.com/world/2020/mar…
Favipiravir inhibits RNA-dependent RNA polymerase, an enzyme that makes copies of the virus’ RNA (not DNA) genome. RNA viruses need this enzyme to make new viral particles, so it’s quite plausible that this drug IS effective against #coronavirus.😃

I worked to develop a similar drug (galidesivir, BCX4430) with the same mechanism of action as #favipiravir, and both drugs show activity against a broad spectrum of different RNA viruses.

Entirely plausible that #favipiravir is active against #COVID19.😃
