We should distinguish between organisations that are inter-governmental & those that have a greater reach. The WTO's head is chosen by consensus by member governments. His/her (one day!) power is limited. The decisions that affect policy are taken by the members by consensus

So what does "democratic" mean among inter-governmental organisations? It should mean among government representatives. Otherwise we are moving into world government. (The UN P5 aside, that's realpolitik.)

Does the UK have an elected representative at the WTO? Yes, the trade minister.

An elected representative at the UN? No. but it has ambassadors under instructions from an elected govt.

Should our UN representatives be elected? Would that mean world govt?
