Short THREAD on the importance of transmission clusters for the spread of #covid19:

(1) Endo, @AdamJKucharski et al. have recently shown that 80% of transmissions seem to be caused by 10% of index cases.

(2) Leclerc et al. looked at what settings have been linked to SARS-CoV-2 transmission clusters (i.e., where these 10% of these “super spreaders” could be found):

(3) They searched the scientific literature for information about #SARSCoV2 transmission clusters and extracted data into a publicly available Google Sheet:

docs.google.com/spreadsheets/u…
(4) Findings A: vast majority of clusters happened indoors, only small number of clusters linked to schools and reported cases there were most often in teachers and other staff, most transmission took place in households...
(5) Fingings B: Settings associated with large cluster sizes were..religious venues, worker dormitories, elderly care homes, hospitals, and ships. Also implicated but to lesser extent: sports settings, bars, shopping venues and conferences.
(6) KEY TAKEAWAYS 1: Clusters most commonly occur in situations that share commonalities with family-settins: groups of people who socialize indoors for extended periods of time, who talk, eat and drink together.
(7) KEY TAKEAWAYS 2: Outside of your own family, avoid combination of 4 C‘s:

1) closed space with poor ventilation
2) crowded with many people
3) conversations and vocalization in close proximity (within arm's reach of one another)
4) consuming foods and beverages
