Read on for some thoughts about hospitalization numbers, vaccine timing, and the predicted baby bust 🚼

2/ @UCSDHealth has hit a temporary equilibrium with census relatively stable ~7 days. Today we have 127 inpatients hospitalized for #COVID19 including 40 in the ICU, 31 on mechanical ventilators, and 7 on ECMO circuits
3/ @SanDiegoCounty hospitalization data shows a similar plateau with 1,591 inpatients across multiple hospitals today and a 7-day rolling average of 1,573 according to @HASDIC data
4/ Remember that current hospitalization represents infections that spread around Thanksgiving (!)

New #SarsCOV2 cases @UCSDHealth are at record highs, confirming that January will be complicated by a "surge on surge" effect, and possibly other factors...
5/ Some of these new #COVID19 cases may prove to be the more contagious UK variant (B.1.1.7) given that several samples have already been identified in @SanDiegoCounty

6/ Identification of this more contagious #SarsCOV2 variant was the result of rapid collaboration between scientists at @UCSDHealth @scrippsresearch @CDCgov as described in this great thread from @K_G_Andersen

7/ Vaccine timing decisions are critical. I have come to the same conclusion as @Bob_Wachter and @ashishkjha - we should consider delaying 2nd dose in favor of rapidly giving more 1st doses. @UofCAHealth leaders will discuss this week.

8/ @UCSDHealth has administered ~8k #COVID19 vaccines to our healthcare employees and students (4,531 @pfizer and 3,429 @moderna_tx)

Unlike this @Forbes report, just 3% of @UCSDHealth employees have declined and 5% postponed vaccination so far

9/ It has now been 42 weeks since the March 13th federal declaration of emergency. As a pediatrician, that means we are now post-term, and might begin to see the "COVID baby-bust" which predicted 300-500k fewer births in the US

10/ The #COVID19 baby bust is not (yet?) evident in the @UCSDHealth newborn census, where births have remained vigorous. Will recheck in a few months

Of course this is not necessarily representative of the entire population and could be a result of market shifts or other biases
11/ Some countries also observed fewer premature births last summer, which was attributed to COVID lockdowns

12/ Slicing @UCSDHealth data over a 2 year time period does not seem to show a reduction in preterm births (<35 weeks gestation) in our local data

Once again, this does not necessarily represent the entire population, and could be an artifact of market shifts or other biases
13/ With record high #COVID19 cases, hospitalizations, and deaths across the US, it is somewhat shocking to hear @WHO leaders share that #SarsCOV2 may "not necessarily be the big one." ⚠️⚠️⚠️

14/ On the bright side, there are a lot of things to look forward to in 2021...

💉 widespread vaccination, allowing

🏫 schools to re-open for in-person instruction, and

🧑‍⚕️healthcare workers to take a well-deserved break!

Let's talk about #COVID19 contact tracing...almost 27k #COVID19 cases in @SanDiegoCounty, and county staff can NOT keep up. Despite hiring >200 contact tracers, just 10% of cases are now investigated within 24 hours!
2/ Contact tracing works great for sexually transmitted infections, but does not scale well for a pandemic caused by a respiratory disease spread by airborne droplets. So what should we do? Hire more contact tracers?

3/ Recent models show that "minimizing contact tracing delays, for instance with app-based technology, further enhanced effectiveness, with the potential to prevent up to 80% of all transmissions."

1/ COVID (@UCSanDiego) Chronicles - July 23, 2020

@UCSDHealth is now caring for 51 inpatients with #COVID19 including 21 on vents and 5 on ECMO. These individuals come from all over southern California, particularly @SanDiegoCounty and @ImperialCntyCA

2/ In a bit of bright news, with just 16 positive PCR results in the last 24 hours, the 7 day rolling average @UCSDHealth decreased to ~21 positive tests/day. This rate of ~2.3% is now the lowest across @UofCAHealth

3/ Unfortunately, @SanDiegoCounty hit two grim milestones yesterday with over 25k #COVID19 cases (+587) and over 500 deaths. San Diegans over 70 yo represent only 17% of the cases but 87% of the deaths, and nearly all had underlying medical conditions.

As we end this holiday weekend @UCSDHealth is caring for 47 inpatients with #COVID19. Overall @SanDiegoCounty bed capacity is fine with 382 total inpatients, though will be watching closely for an ↑ in based on recent data.
2/ The amazing @UCSDHealth #SARSCoV2 results team is monitoring >100 patients at home or in public health temporary housing. This is the highest # of people monitored in home isolation in a single day. It is also 10x higher than baseline monitoring responsibilities 6 weeks ago.
3/ #SARSCoV2 test results from July 4th, 2020 shows another record setting day with 562 new cases identified in @SanDiegoCounty and 5,410 new cases in California.

Lots of news commentary today but first, data: 28 inpatients @UCSDHealth for the second morning in a row (8 on vents); the relatively steady number belies busy admit/discharge activity
2/ @UCSDHealth hit a milestone yesterday with over 10k tests performed. Yesterday just 4 of 540 #SARSCoV2 PCR results were positive, and serology results still ~2%. @SanDiegoCounty added 131 new cases for a total of 3,842.
3/ On the news front...a solid vaccine candidates comes from @ArcturusRx here in @SanDiegoCounty but there's a catch - development was funded by @govsingapore and first doses will go to Singapore. nbcsandiego.com/news/local/pot…
