Young Man in whom a Healthy Habit caused erectile dysfunction
1. 30-year-old Rijo consulted me for 3-month history of problem with erection. He was evaluated by Urologist/Andrologist & no cause for the same was found. Therefore, he was referred to rule out neurological disease.
2. Rijo was well three months ago, when he noticed erectile dysfunction (ED). He also had numbness in penis and surrounding regions, which was worse in mornings and better in evenings. There was no back pain or leg pain.

3. On enquiring about his fitness regime, he mentioned that he was passionate about #cycling and he cycled about 400 km per week (including 100+ km on Sundays). I could now guess the reason for his ED.
4. I did not order any investigations, nor did I prescribe any medicines.
I offered him a few tips: sit on a no-nose saddle while cycling, stand on the pedals & cycle every 10 minutes (while cycling), reduce the mileage to 300 km or lesser per week.
5. At one month review, Rijo reported significant improvement- numbness in perineal region had reduced and his sexual functions were much better. *The final diagnosis was: ED and perineal numbness caused by excessive cycling.
6. The mechanism of perineal numbness and ED in cycling is compression of nerve and blood supply to perineal region.
7. Tips to avoid ED and perineal numbness among cyclists-
*Limit cycling mileage to 300 km per week,
*Use a no-nose saddle,
*Cycle with standing on the pedals every 10 minutes (for a few minutes).
8. Cyclists have a 2-fold higher risk of erectile dysfunction.

10. 19% of cyclists with weekly training distance >400 km complained of erectile dysfunction. Numbness in genital region was reported by 61% of cyclists. The study showed that there is a deficiency in penile perfusion due to perineal arterial compression.

11. Cycling can adversely affect women too.
Genital numbness is common in women who cycle and is associated with increased risk of Female sexual dysfunction. Higher handlebar height was associated with less numbness.

Consider lead toxicity due to Ayurvedic medications in patients with unexplained abdominal pain

1. In the past 6 months, we saw 5 patients with unexplained abdominal pain associated with weakness of hands and feet. All were taking Ayurvedic medicines for various reasons.
2. Blood lead levels were high in all of them (>25 mcg/dl). There was no history of occupational lead exposure in any of them.
They were treated with d-penicillamine (chelation therapy). Ayurvedic medicines were stopped.

Sharing the inspiring journey of Mrs Ravi

1. Mrs Ravi to me: "Dr, you told me if I lose 10 kg weight, 50% cure can be achieved. I thought why not lose 20 kg and aim for 100% cure". This was about two years ago. She came for review today.

1. Arti to me "Dr, when will my husband Anuj start talking & walking?" Her two kids aged 3 & 5 were holding her hands, as Anuj was brought into my OPD for review one month after suffering brain hemorrhage. Anuj had become paralysed on right side and lost his speech due to stroke

Taking frequent walking breaks during prolonged sitting could help lower blood sugar and BP

1. Sedentary lifestyle with prolonged sitting hours (>8 hours daily) is common these days.
Frequent walking breaks (1 min or 5 min every 30-60 minutes) could help lower blood sugar and BP
2. When participants took a 5-minute light #walk after every half hour of sitting, the blood #glucose spike after eating was reduced by 58% compared with sitting all day (reduction one would see in a diabetic using insulin or #diabetes medications to control their blood sugar).
3. The largest reductions in systolic blood pressure were observed when the walking break was 1 minute every 60 minutes (-5.2 mm Hg).

Anti-acidity medicines increase the risk of heart attacks

1. Proton pump inhibitors (PPIs) like omeprazole, esomeprazole & rabeprazole are commonly used for treating GERD and other acidity-related disorders.
A recent study looked at long-term PPI use and risk of heart diseases.
2. 19,229 adults with type 2 diabetes were followed up for about 11 years.
Long term PPI users had 27% higher risk of coronary artery disease; 34% higher risk of myocardial infarction, 35% higher risk of heart failure & 30% higher risk of all-cause death, as compared to non-users
3. The mechanism of PPI increasing the risk of #heart diseases could be: the effects of PPI on altering gut microbes and their interaction with anti-platelet agents (PPIs may reduce the bio-availability of clopidogrel).

Is direct discharge to home (DDH) from ICUs safe?

1. Patients are usually transferred to hospital wards from the ICUs and discharged to home after another few days. However, there could be delay in getting a ward bed or patient might have significantly recovered while in ICU.
2. Moreover, some families find it difficult to afford longer duration ICU (& hospital stay) costs. This necessitates direct discharge to home (DDH) in some patients. In a recent study, the safety of DDH was evaluated and compared to those who were transferred to wards from ICUs.
3. For DDH from ICU, compared with ward transfers, there was no difference in the risk of-
*Emergency department visits at 30-days (22.4% vs 22.7%);
*Hospital readmissions at 30-days (9.8% vs 9.6%); or
*90-day mortality (2.8% vs 2.6%).
