Young woman with progressive muscle weakness was suspected to have muscular dystrophy, but a clinical finding pointed to another diagnosis
1. 22-year old Mini had weakness of legs and arms for two years. Weakness began in legs and after a year involved the arms too. #MedTwitter
2. Mini had difficulty in getting up from squatting position, climbing stairs, combing hair & reaching for top shelves in kitchen.
She also had myalgia, muscle cramps and fatigue.
She had no numbness and had normal bladder & bowel control.
No one else in family was affected.
3. Mini consulted a local physician, who ordered a few investigations. Serum CPK was high (2600 U/L, normal<190 U/L). EMG showed low/small amplitude potentials suggestive of myopathy. He suspected a diagnosis of Becker's muscular dystrophy & referred for muscle biopsy to confirm.
4. I reviewed Mini's history & performed clinical examination.
First observation was bulkier calves (which can be seen in Becker's muscular dystrophy).
Second was delayed relaxation of ankle reflex (hung up reflex). This made me suspect another diagnosis.
5. I tried to elicit a specific clinical finding. I tapped on arm muscle with a percussion hammer. The muscle belly formed a lump rising from the surface (mounding) and the lump disappeared after 30-40 seconds. This finding is called myoedema.
Now, the diagnosis was clear.
6. I ordered for thyroid profile- reports showed high TSH, low T3 and T4 levels- confirming hypothyroidism.
Now, muscle biopsy was not needed.
Final diagnosis was hypothyroid myopathy. Mini had no other clinical features of hypothyroidism. japi.org/s2e4a444/myopa…
7. Mini was started on thyroid hormone supplements. Serum CPK level reduced to 1220 U/L after 4 weeks. TSH level reduced (from 125 to 35 mlU/L) by 3rd month. Muscle strength gradually increased over several months and she had near normal muscle strength after one year follow up.

2. Between 2015 & 2021, the mean sugar content of soft drinks sold in Germany decreased by only 2% from 5.3 to 5.2 g/100 mL, falling short of 9% reduction target. The onus of reducing sugar content was left to the manufacturers of soft drinks.

2. The weakness increased over the next few weeks and his left foot was unable to clear the ground while walking. It used to "slap" the ground while walking, which Hari described as "foot drop". He had no weakness of any other part of body. He had mild numbness over left foot.
3. Hari consulted an orthopedic surgeon, who concluded no significant abnormality on ankle and knee X-rays and referred him to a neurologist. The neurologist performed nerve conduction studies and told that there is damage to a nerve in leg, most likely due to fall while cycling

1. 51-year old Mr K presented with three episodes of numbness of left side of face, arm and leg in the past 20 days. Each episode lasted only 10 minutes. #MedTwitter
2. Mr K also had feelings of tingling sensation in the affected areas. He had no weakness and could move his left arm & leg normally during those episodes. His speech also remained normal.

3. Mr K was a healthy person, with normal weight and BMI. He had no known medical illness. However, he consulted me for expert opinion.
The history was very typical of a neurological problem. Clinical examination was normal. I asked for appropriate investigations.
1. The incidence of PD has been increasing. Known risk factors such as genetic and head injury can't account for this increase in PD.
Exposure to TCE could be one of the factors causing an increase in PD.
2. TCE is a simple, six-atom molecule that can decaffeinate coffee, degrease metal parts, and dry clean clothes.
A small epidemiological study found that occupational or hobby exposure to the solvent was associated with a 500% increased risk of developing PD. #MedTwitter
