The doctor's name on the form is Dr John Houghton of Brownlow Group Practice, 70 Pembroke Place, Liverpool L69 3GF. The Prescriber Number given is 803602. The serial number of the FP10 is 59533380592. Dr Houghton is listed on the Practice website: central.brownlowhealth.co.uk/staff/.

The script, dated 27 December 2019, purports to be written for the comedian and writer Mr Graham Linehan (@glinner) and was posted in a reply to his Tweet on 26 December 2019:

The FP10 has his correct date of birth, gives an NHS number of 999 999 9611 and EMIS number of 48422. The address given is, '10 Terf Street, Transphobia'. Since Graham Linehan lives in Norwich, it is unlikely he is a member of that practice.

Frequently used against women, 'terf' is a demeaning and insulting term frequently used to silence and harass. For further details, see terfisaslur.com

The treatment prescribed is to 'Log Off'. Whilst this might be seen by some to be light-hearted, it could also be seen as harassment by the poster, 'Gemma' and possibly the person who created it or allowed the FP10 form to be used for these purposes.

It is also concerning that whoever created this prescription had access to, or was supplied with, the FP10 from that practice.

A further Tweet dated 13 January 2020 also included the FP10 and states that 'Gemma' created it 'as a joke'.

It is not clear whether this photograph has been manipulated in a graphics program such as Photoshop or whether it has been completed and printed out at the Brownlow Group Practice.

However it was produced, it is concerning that it was produced, that it identifies a doctor and the practice and is clearly an abuse and misuse of NHS resources. It is not a joke.

That is was used to harass Graham Linehan is not behaviour expected of any NHS employee and could be seen as bringing the profession, the Practice and the NHS into disrepute.
