It is an enormous pleasure to be part of this #paper: #COVID-PIEL, the Spanish muticentric study of the #cutaneous manifestation of #COVID19 has just been published in the prestigious journal British Journal of Dermatology! 375 patients included! Conclusions here 👇 (#thread)
A new classification of #skin manifestations of #COVID19 is proposed: 1. Acral areas of erythema-oedema with some vesicles or pustules (19%); 2. Other vesicular eruptions (9%); 3 Urticarial lesions (19%); 4. Other maculopapules (47%) and 5. Livedo or necrosis (6%)
1⃣ Acral lesions, #COVIDtoes, pseudo-chiblains..., affected young patients and lasted a mean of 12,7 days to appear in the course of COVID. They were associated with less severe disease. Itchy (30%) or painful (32%).
2⃣ Vesicular monomorphic lesions (not chickenpox-like, that are polymorphic) appeared in middle-aged patients and, more commonly, before other symptoms. They were associated with intermedium severity. Itchy (68%)
3⃣ Urticarial and 4⃣ maculopapular lesions appeared at the same time than the rest of the symptoms and were associated with more severe #COVID19 disease (2% mortality in the sample). Itchy.
5⃣ Livedoid/necrotic lesions took place in older patients with more severe disease (10% mortality). Severity of associated disease followed a gradient, from less severe disease in pseudo-chilblain to most severe in patients with livedoid presentations.
Conclusions: 1⃣ Pseudo-chilblain may look like perniosis, and as they appear later in the evolution and are less
commonly associated with virologic confirmation; 2⃣ Monomorphic disseminated vesicular lesions and acral vesicular-pustulous lesions are probably quite specific
3⃣ Most of the urticarial and maculopapular lesions might not be very helpful for diagnosis, as these are common and may have many different causes and, 4⃣ Livedoid and necrotic lesions were relatively uncommon, and mostly appeared in elderly and severe patients.
