VERY WORRIED—Runaway train #P1 variant still uncontrolled in British Columbia 🇨🇦—surging 57% in just 4 days! In contrast, other contagious 🇬🇧variant #B117 only up 17%. #P1 from🇧🇷 is outcompeting #B117 by absolute increase too! BC must act! #bcpoli #COVID19

2) #P1 clearly a crisis in BC. But notice it’s relatively new uptick in Ontario and Alberta too. This is a bad sign.
3) it is incumbent on BC Canada 🇨🇦 health leaders (@DrBonnieHenry @adriandix @jjhorgan) to contain #P1 immediately. Or else it endangers not only rest of Canada but also North America and maybe even rest of the world. #P1 is ~2-2.5x more transmissible—the fastest variant to date.
4) Remember these olden days when BC 🇨🇦 only had 10-20 new #P1 cases a week? BC just recorded 555 new P1 in just 4 days now (top graph).
5) And yes, P1 not only more transmissible but also has some reinfection risk according to models from a leading UK expert group.
6) How good is are the mRNA vaccines against this #P1 variant? Only semi— it has moderate neutralization- better than the SA #B1351, but worse than #B117 (which responds well to vaccines overall). Note this is lab neutralization not direct % efficacy.

7) as for J&J—we don’t have direct evidence for #P1 but I performed decently in both South Africa and in Brazil for severe disease. It was conducted in SA during the surge of #B1351. So hopefully J&J decent for P1 too.
8) What about Brazil & Chile’s main vaccine from SinoVac- called CoronaVac? Well not direct evidence but it performed okay in Brazil with 50%. Though authors note it was before #P1 emerged where their trials were.
9) The closest #P1 efficacy study for CoronaVac is this one from Manaus. Not a trial, but it suggests 50% efficacy for symptomatic and 35% for asymptomatic. See thread 🧵
10) Meanwhile in Ontario, the #B117 hotbed of Canada, its #COVID19 hospitalizations are so serious, doctors who don’t work ICUs are being asked to transfer to ICUs to “avoid total collapse”.

Now imagine the crisis in the rest of the world with much less wealth and resources.
11) We see the worrisome #P1 >> #B117 signal again in Netherlands 🇳🇱. Despite B117 nearing complete domination, #P1 is still making inroads.
12) I’m having deja vu bad feeling again... In addition to BC 🇨🇦 and Netherlands 🇳🇱 all having #P1 surges (besides Brazil 🇧🇷 of course), UK just officially recorded #P1 as the fastest growing variant in UK as well. TRIPLE CONFIRMATION of this disturbing trend!

2) TRANSLATED: A nurse at the Albert Schweitzer Municipal Hospital in Realengo, in Rio's West Zone, says that patients with the most severe form of the disease are intubated, wokenand tied to beds due to the absence of medications.
3) Without the medication, the teams unfold looking for alternatives.
According to intensivist physicians, mechanical restraint can be used, but the patient needs to be medicated with at least a mild sedation.

MIXING SUBSTITUTE VACCINE—Germany 🇩🇪 has just approved a new move to give people aged <60 who have been given a first dose of the Oxford-AstraZeneca #COVID19 vaccine a different jab for their 2nd dose— approving Pfizer & Moderna for 2nd dose as substitute.

2) Germany announced on March 30 that it would no longer offer the two-dose AstraZeneca vaccine to people aged under 60 due to concerns over a possible link to rare cases of blood clots.
3) DPA news agency said ministers agreed at a meeting that people in the younger age group who received a first AstraZeneca dose before the March 30 announcement will be offered either the BioNTech-Pfizer jab for their second dose, or the Moderna vaccine.
