⭐️The new C. difficile guidelines! Image
Here are the 🧠s behind the new guidelines! Image
What's in a name?

Clostridium difficile ▶️ clostridioides difficile Image
Community acquired CDI is on the rise. Image
Diagnosis can be tricky!

GDH is sensitive but not specific
Toxin EIA is specific but not sensitive
PCR is sensitive, but not specific ImageImageImage
Watch out for symptomatic colonization (+PCR)

PI-IBS occurs in about 25% of cases treated for CDI Image
Sensitive ▶️ specific Image
Metronidazole: cheapest
Oral vancomycin: 10x more $$$ than metronidazole
Fidaxomycin: 30x more $$$ than metronidazole; most narrow spectrum; prevents recurrence the best Image
Image
Non-severe initial episode: vancomycin or fidaxomicin

IDSA: never use metronidazole
@AmCollegeGastro: can use metronidazole in low-risk patients Image
Severe initial episode: vancomycin or fidaxomicin

⛔️metronidazole Image
Fulminant: fluids, surgery consult, imaging, high dose vancomycin AND metronidazole

With ileus, consider vancomycin enema

If no response in 24-28 h, consider ✂️

New recommendation: can consider multiple (avg 3) #FMT +vanc for 1st episode ImageImageImage
Pregnant: vanc, no metronidazole
Immunocompromised: vanc or fidaxo

#IBD: vanc for MINIMUM 14d; consider #FMT for rCDI ImageImage
First recurrence: fidaxomicin or tapering/pulse dosed vancomycin Image
NNT = 3 for #FMT preventing CDI recurrence

Can use FMT at second recurrence (3rd episode). Colonoscopic or "crapsule." Repeat FMT if recur within 8 weeks of FMT ImageImageImage
There is no role for #probiotics in the primary or secondary prevention of CDI; the guidelines explicitly recommend against.

#microbiome Image
Suppressive oral vancomycin in patients who are not candidates for #FMT or fail FMT

OVP can be considered in high risk patients getting antibiotics Image
Bezlotoxumab: consider in those with high risk of recurrence.

Careful in patients with heart failure Image
Do not discontinue PPI (unless they don't need in the first place!) Image

I gave a talk on inpatient bowel regimens today for the @YaleMed @YaleIMed residents and students, with additional 💎s on outpatient #constipation. Here are the slides!

Most inpatients require a bowel regimen. Our patients typically have most of the risk factors for #constipation!
#Constipation is more about symptoms than frequency! 🚽

It’s also a *symptom* (like chest pain), so need to figure out what’s causing it and treat accordingly.

💨The gut microbiota generates ~0.2L–1.5L of gas per day
💨>99%: hydrogen (H2), carbon dioxide (CO2), & methane (CH4)– *odorless*
💨foul smell of gas from <1%: sulfur–containing gases hydrogen sulfide (H2S), methanethiol (CH3SH), dimethyl sulfide [(CH3)2S] + volatile compounds
🦠bacteria in the colon ferment substrates that are not digested or absorbed in the upper GI tract (e.g. carbohydrates, proteins, fats)
🦠produce short-chain fatty acids, branched-chain fatty acids, ammonia, N-nitro compounds, phenolic and indolic compounds, and various gases
