🚨Delighted to do a quick #tweetorial on our short report in the💚#GreenJournal @RadiotherapyOn1

☢️Heterogeneous immunogenicity of SARS-CoV-2 vaccines in #cancer patients receiving
#radiotherapy💉

Inspired by:

[Our patients]
Doctor, is the #radiotherapy I am getting affecting how well my #CovidVaccine works?

[Us]
🤷
3/

There is large heterogeneity when looking at immunogenicity of #COVID19 vaccines in #cancer patients & esp #lungcancer patients shown by @vivek_naranbhai & team in @JCO_ASCO

Chemo may suppress antibody levels somewhat.
What about #radiotherapy?

➡️ascopubs.org/doi/pdf/10.120…
4/

In a smaller but unique cohort of #lungcancer patients receiving #CovidVaccine shortly before, after, or during thoracic #radiotherapy, antibody levels were lower than in non-radiation (RT) patients or health controls (HC) 🤔

sciencedirect.com/science/articl…

@sueyom @VPrasadMDMPH
5/

However, 55% of patients had potentially immunosuppressive conditions = recent receipt of chemotherapy, chronic kidney disease, CLL, immunosuppressive medication🤔

These had lower Ab levels🥀

🚨The other #radiotherapy patients had Ab levels comparable to healthy controls🚨
6/

➡️This needs to be validated in larger cohorts & different #radiotherapy patients

Is it possible that large field #radiotherapy could affect #CovidVaccine immunogenicity via lymphopenia or impacting lymph node function⁉️

@CGrassberger @StevenLin_MDPhD @vivek_naranbhai
7/

4 received additional #CovidVaccine doses with favorable Ab response especially in those who had initially low levels

➡️However what will be the duration of response in immunosuppressed people❓

➡️This data highlights usefulness of measuring Ab titers in #cancer patients🚨

Last but not least:

Thanks to @JustinGainor @vivek_naranbhai @MGHThoracicOnc @BalazsLab & whole team for making this happen🙏

Thanks to our patients for participating🙏💕

We learned a lot actually + it helped our patients😊

So thrilled about #RadRes2022 Jack Little Legacy Plenary Lecture featuring 2019 Nobel Prize Laureate Bill Kaelin📢🔥

Also glad to snatch #Twitter selfie with @DKirschMDPhD who is introducing the speaker🤗
2/
#RadRes2022

Dr Kaelin starting off with a few pearls of wisdom:

1️⃣ He advises that he actually does not have a PhD🙃

That is good news for some of us that this not a principal #physicianscientist career killer😁

(Though it probably does not hurt 😅)
3/

#RadRes2022

2️⃣also, keep believing, and disregard predictions like this one from one of his prior advisors who questioned his career trajectory 😁 #NobelPrize

3️⃣Great #Mentorship is key⚠️

💊How to best combine targeted agents with #radiotherapy☢️

Ch4⃣from our book
springer.com/us/book/978303…

Biomarkers of clinical radioresistance. Great pleasure to have written this one with @max_diehn @michaelsbinkley Iris Eke @StanfordRadOnc🙏

#radonc #radbio #RTdrugcombo

As @SorenBentzen keeps saying, absolute radioresistance does not exist. You just have to up the dose💪

That is very different from drug response, which is a yes/no situation - where more drug does not give you typically a better response (with caveats 😉)

#radonc #radbio
3/n

Tumors can have variable radioresistance. Lab & human data indicate that relationship between dose + tumor eradication is sigmoid🚨

Really high doses are prohibited by tolerance of normal tissue surrounding the tumor‼️

= Rationale for pursuing targeted radiosensitizers💡
