1/n September Pulmonary Fibrosis controversy thread about #IPFandGERD preprint from Dr. Cosetta Minelli's group: Two tweets from really good people say "GERD Causes IPF" (@IPFdoc, @drcjar), and a 3rd (@LungDrE) says: "Not based on your data".

Who is right?
2/n I read the preprint (medrxiv.org/content/10.110…); A Mendelian Randomization (MR) study using summary stats of GERD GWAS gut.bmj.com/content/71/6/1… & IPF GWAS meta-analysis thorax.bmj.com/content/77/8/8….
Shows OR of 1.6 GERD➡️IPF & 0.99 IPF➡️GERD. So what do I think?

#IPFandGERD
3/n #IPFandGERD Paper obviously needs #PeerReview. But there is context.
As a non-expert, in reading Mendelian Randomization studies I go back to the superb Guide to reading Mendelian randomization studies from Prof Davey Smith & team. Especially that 👇🏼
bmj.com/content/362/bm…
4/n Prof Davey Smith highlights need to triangulate the data. Using observational data, REPLICATED Mendelian randomization Trials, intervention studies, to infer causality, recommend action. Methinks that;s the answer to @IPFdoc, @drcjar @LungDrE #IPFandGERD causation argument
5/n Do we have the data to triangulate?
Maybe. Results of #WrapIPF testing laparoscopic anti-reflux surgery in patients with proven #GERDandIPF were as good as recent phase 2A studies (see 🧵 from @davidlederer)

but do we have enough to infer causation?
6/n there's much more (dovepress.com/idiopathic-pul…), but #TruthIs @LungDrE is RIGHT. The Reynolds/Minelli #IPFandGERD study does not prove GERD causes IPF. But it does suggest that the reverse causality - IPF causing GERD probably does not happen. New & VERY cool if replicated. Fin

1/n So, what do I think, as a non-expert, son to holocaust survivors, about @VPrasadMDMPH's article, claiming #COVID19 restrictions could be used as blueprint for a future totalitarian regime, his analogy to Hitler's rise in Germany. @GYamey says it better
2/n Frankly, not much. My approach is, if you want to understand threats to democracy go to experts (I am not, @VPrasadMDMPH is DEFINITELY NOT) there are really good books, like @TimothyDSnyder's #OnTyranny. Check out this clip
3/n If you want to understand the road to Fascism, go to the experts like @jasonintrator (@VPrasadMDMPH is REALLY NOT an expert). Listen to this interview to understand real threats. Read @jasonintrator's book #HowFascismWorks. penguinrandomhouse.com/books/586030/h…

1/n This thread is not about Medicine, Science or Lung Disease. It’s about what you should ask when when you hear about another cycle of violence in #IsraelPalestine. Written in #Solidarity with the Jews & Arabs who today stand together against racism & hate #TrueHeroes
2/n When people speak about “country x’s right to defend itself” - what you should ask, does the leadership of country x have the right to provoke violence? and do the actions taken really constitute defense or serve another agenda? #KnowWhatToAsk #IsraelPalestine
3/n When they say but “look at the violence in the streets by the minority ” - what you should ask, is what has been done to the minority , how many have been dispossessed, killed without recourse, disenfranchised and for how long? #IsraelPalestine #EndTheOppression

1/n Usually we invite friends/colleagues to our SEDER, but no guests last & this year. Inspired by FB comment by @gregggonsalves I thought it’d be fun to share our dishes with #MedTwitter friends as if we’re together - see following thread #PCCMEats #HappyPassover2021
2/n Every item on the Seder Plate has a symbolic meaning in the ceremonial retelling of the Exodus story - except the Orange, a recent addition dedicated to #inclusion of those marginalized in traditional religions (women, LGBTQ) #NKsSederTweetorial #PCCMEats #HappyPassover2021
3/n The bitter & spicy taste of horseradish, fresh taste of greens & Sweet & earthy look of Harosset all symbolize aspects of the story - but biggest mystery is how well they fit together in a Matzah sandwich - or is it not? #NKsSederTweetorial #PCCMEats #HappyPassover2021

1/n Our Collaborative "Single-Cell Omics Reveals Dyssynchrony of the Innate & Adaptive Immune System in Progressive #COVID19" paper is now on @medrxivpreprint. We provide @10xGenomics #scRNAseq, #Cite, #TCRseq & #BCRseq: my nonimmunologist's tweets follow medrxiv.org/content/10.110…

1/n The #IPFCellAtlas is finally out! We profiled 312,928 cells from 32 IPF, 28 controls, 18 COPD lungs using @10xGenomics #scRNAseq & identified previously unidentified populations of epithelial & endothelial cells enriched in IPF #CureIPF advances.sciencemag.org/content/6/28/e…
2/n Among epithelial cells, we identified cells DISTINCT from ANY epithelial cells previously described in human lungs. These aberrant basaloid cells are at the edge of myofibroblast foci & express fibrotic molecules & regulators #IPFCellAtlas #CureIPF advances.sciencemag.org/content/6/28/e…
3/n Among enodthelial cells we discovered a previously undescribed population of COL15A1+ CD31+ cells. They are usually restricted to the airway but in IPF are found in the lung parenchyma - what are they doing there? #IPFCellAtlas #scRNAseq #CureIPF advances.sciencemag.org/content/6/28/e…

1/n Dr. Mark Siegel is a superb critical care physician, ethicist & legendary Internal Medicine Residency Program Director whose weekly notes I read eagerly. This week’s note is titled “what I’d say to new interns” #MarksWisdom #TipsForNewDocs #MedEd

2/n Siegel starts saying #COVID19 took away joys of orientation: pizza, hot dogs, cold beer, gelato, unscripted conversations & new friendships. But don’t worry, show up, work hard, don't worry, you'll do great & then follows with 14 SPECIFIC TIPS: #MarksWisdom #TipsForNewDocs
3/n Tip #1: Take your own histories. Charts are rife with inaccurate, missing information. Key exposures, mounting symptoms, details patients held back- until you asked. To paraphrase Osler, patients will tell you the diagnosis, if you listen. #MarksWisdom #TipsForNewDocs #MedEd
