Repeated short-duration numbness of one side of body: prompt management can prevent a disabling disease

1. 51-year old Mr K presented with three episodes of numbness of left side of face, arm and leg in the past 20 days. Each episode lasted only 10 minutes.

2. Mr K also had feelings of tingling sensation in the affected areas. He had no weakness and could move his left arm & leg normally during those episodes. His speech also remained normal.
After 10 min, he used to have complete recovery from numbness and tingling.

3. Mr K was a healthy person, with normal weight and BMI. He had no known medical illness. However, he consulted me for expert opinion.
The history was very typical of a neurological problem. Clinical examination was normal. I asked for appropriate investigations.
4. Blood tests revealed the following abnormalities: Very low vitamin B12 and D, high homocysteine, high total and LDL cholesterol; high triglyceride. HbA1C level was normal.
These abnormalities were all risk factors for the disabling neurological disease, I was suspecting. ImageImage
5. MRI brain was normal. MR Angiogram showed narrowing of right posterior cerebral artery (PCA). Now the diagnosis was confirmed: Recurrent pure sensory transient ischemic attacks (TIAs).
If left untreated, many of these patients can develop #stroke.
6. Pure sensory TIAs strongly suggest the presence of PCA disease. Repeated compromise of small vessels supplying the posterior-lateral part of the thalamus seems to be the pathogenic mechanism.

7. I started Mr K on appropriate medical treatment, including antiplatelet and statins. Also counselled him about #healthylifestyle, #sleep and #exercise .
8. Take home message
Repeated episodes of stereotyped numbness or tingling sensation affecting one half of the body could be due to compromised blood supply to right side of brain (thalamus) and could be suggestive of TIA. Prompt evaluation and management can prevent stroke.
Mr K was a strict vegetarian and his vitamin B12 was very low (20; normal in our lab is >70). As there are no good sources of vitamin B12 in vegetarian diet, I advised vitamin B12 supplements to him.

Acute effects of coffee consumption on health

Young man had undiagnosed neurological problem; skin examination revealed the diagnosis

1. 30-year old Hari presented with weakness of left foot for six months. He attributed his symptoms to a fall while cycling a week prior to the onset of symptoms.

2. The weakness increased over the next few weeks and his left foot was unable to clear the ground while walking. It used to "slap" the ground while walking, which Hari described as "foot drop". He had no weakness of any other part of body. He had mild numbness over left foot.
3. Hari consulted an orthopedic surgeon, who concluded no significant abnormality on ankle and knee X-rays and referred him to a neurologist. The neurologist performed nerve conduction studies and told that there is damage to a nerve in leg, most likely due to fall while cycling

Trichloroethylene (TCE): An invisible cause of #Parkinsonsdisease (PD)

1. The incidence of PD has been increasing. Known risk factors such as genetic and head injury can't account for this increase in PD.
Exposure to TCE could be one of the factors causing an increase in PD.
2. TCE is a simple, six-atom molecule that can decaffeinate coffee, degrease metal parts, and dry clean clothes.
A small epidemiological study found that occupational or hobby exposure to the solvent was associated with a 500% increased risk of developing PD.

3. Exposure is not confined to those who work with the chemical. TCE pollutes outdoor air & groundwater, and contaminates indoor air. The molecule, like radon, evaporates from underlying soil and groundwater and enters homes, workplaces, or schools, often undetected.

Perceived stress increases the risk of cognitive decline and dementia

1. A recent study looked at the association of perceived stress and incidence of cognitive impairment in a long-term follow up study

2. 24,448 people (median age 64 years) were followed up for >15 years. Elevated levels of perceived stress (dichotomized as low stress vs elevated stress) was associated with 1.37 times higher odds of poor cognition.
3. The association of the change in the Perceived Stress Scale score with incident cognitive impairment was significant in both the unadjusted model (OR, 1.62) and after adjustment for sociodemographic variables, cardiovascular risk factors, and depression (AOR, 1.39).

"Burn-out" is real- take care to prevent it

1. Dr S was a very sincere pediatrician. She would spend quality time to counsel parents and clear all their doubts. She would attend to every case referred to her in emergency room (ER) and spend at least an hour per patient.
2. Dr S had a beautiful handwriting and her case notes were detailed. For a child with viral fever too, she would write two page notes and counsel parents for one hour (even if it was past midnight). Her work was admired by parents (of sick children) as well as colleagues.
3. Within 3 months of joining the hospital, Dr S became very popular and became busy in OPD from 8 AM to 7 PM. In addition, she got calls from ER almost on daily basis and spent 3-4 hours every night attending to emergency patients. This continued for a year or so.

Drinking hot tea could increase the risk of esophageal cancer
