A few weeks ago I noticed that one of the on-call doctors didn't seem quite himself.

We're not friends outside of work + I'm not his supervisor, but he works in an acute specialty so is regularly in the ED seeing patients + that day something just wasn't quite right.

I asked him if he was ok and he replied, "Yes, I'm fine", but I wasn't convinced.

Later on I spoke to him again - I said that I had noticed a change and was worried about him.

This time he told me everything... things he hadn't told his colleagues, his friends or his GP.

Reframing the question really changed the response I got and we then spoke about what had been happening, how he was feeling + how it was affecting him and his work.

Together we were able to make a plan to help him move forward.

One cold night in winter of 2003 I was Chief Resident @RCHMelbourne & I was asked to give a talk to potential paed residents...there were also some of that year's residents & Director of Physician Training @margot_nash pushed them: "surely there's some bad things about paeds?"
One particularly great resident - who had gone on to amazing things and just completed his PhD- talked about how much he loved the job but he said:
"I do drive to work each day and know I hurt children"

That changed the course of my career.
He was right.

In 2003 that was still the elephant in the room of paediatrics.

In the years that followed a huge amount of work happened around the world.

And in Australia we had the RACP guideline statement on proc pain:

A few weeks ago, someone from the @GatesUS reached out on here and invited me to a thing. The timing fit perfectly into my work travel schedule, the request came with zero strings, and I was feeling adventurous, so I said yes. It was #NSIYear1. Thread incoming. 🧵
I didn’t livetweet, mostly because I wasn’t sure which hat I was wearing. (Again, no strings.) I did, though, notice things. First, there were zero manels. Several of the panels had more people of color than white members.
The opening speaker, @BobHughesK12 , spoke about the foundation’s openness to feedback. I shared I believed him with two different people during a break and both provided immediate examples affirming my hunch. No lies detected.

One of the main imprints I talk about is the conditioning that comes when there is a discrepancy between what you are feeling/knowing intuitively and what your caregivers, parents, teachers (external authority) are saying is true or real.
Programming you received as a child taught you to trust what external authority (caregivers) told you over your own guidance. To disassociate from your feelings, gut wisdom, heart knowing and intuition. It also taught you to value intellect but to ignore subtle inner messages.
If youre someone who has a pattern of ignoring your own guidance a simple technique I love is to keep an intuition journal Say, show me what I need to know today. Keep a journal of things that leave an impression on you. Even if they seem insignificant logically write them down

I have spent a year reading emergency room bills. I've read about 1,200 of them.

A few months ago, I noticed that one hospital had bills that were *really* different from the rest: Zuckerberg San Francisco General. (1/11)

Zuckerberg San Francisco General is the largest hospital in the city.

It is San Francisco's only level one trauma center.

But it is not in-network with any private health insurance plan. (2/11)
This is *really* unusual. Like I said, I have read hundreds of ER bills.

Sometimes doctors are out-of-network. But the ER itself is almost always in-network.

That makes a lot of sense: patients often don't get a choice in where to seek care in an emergent situation (3/11)
