FUNGEMIA INFECTION & LUNG EMBOLI BY MALASSEZIA SYMPODIALIS IN THE PORTACATH VENOUS ACCESS SYSTEM

The portacath was removed, & the patient was given liposomal amphotericin B, for 4 weeks.

His symptoms were gone by week 2, and the interstitial infiltrates were nearly resolved by week 4.

Systemic infections are uncommon.
The first case of a Malassezia spp catheter-related infection in the bloodstream reported (1981): in a premature infant receiving total parenteral nutrition with lipids with diffuse, bilateral lung infiltrates

Malassezia infection in the bloodstream can be associated with lipid infusions & diffuse pulmonary infiltrates
If Malassezia infection is suspected, the microbiology laboratory should be notified to add specific lipids (ie, olive oil) to the culture media.

Treatment usually requires removal of the catheter and, if a systemic infection is suspected, antifungal therapy.
Most patients have been treated with amphotericin B preparations.

Malassezia are part of the normal human cutaneous flora, & are associated with cutaneous rashes, such as:
✔️pityriasis versicolor,
✔️seborrheic dermatitis, and
✔️folliculitis .

Bullous tinea, seen in adults and children, and most often affecting the feet, presents with an erythematous scaly rash with serous fluid-filled bullae, as opposed to the pus-filled blisters of bullous impetigo.

Mystery #hepatitis cases in #kids linked to unexpected viral suspect

Two preliminary studies finding #adeno-associated viruses in pediatric hepatitis cases suggest a child's genetic background and other virusesall act in concert
1/

science.org/content/articl…
Researchers found high adeno-associated virus 2 (AAV2) levels in the blood or liver cells in 24 of 25 children with unexplained hepatitis.

In a group of kids without this condition, almost none had AAV2, even those with #adenovirus.
2/
The young #hepatitis patients were also much likelier to have a #genetic variant that may make their immune systems overreact to viruses.
