Folks—J&J vaccine is not “inferior”. It gives 100% protection against deaths just like Pfizer and Moderna ones. 72% in US overall—but was tested in a different era with more variants. And faster deployment for poorer areas, but worries about perception. 🧵washingtonpost.com/health/2021/03…
2) “Decisions to send the shots to harder-to-reach communities make practical sense, because j&j single-shot vaccine is easier to store/use. But they could drive perceptions of a two-tiered vaccine system, w/ marginalized communities getting what they think is inferior product.
3) “The issue came up on a recent call between governors and Biden administration officials coordinating the country’s coronavirus response. Gov. Charlie Baker, stressed the need for prominent health officials to communicate clearly about the benefits of the one-shot vaccine
4) “The Johnson & Johnson vaccine proved safe and effective in a clinical trial, completely preventing hospitalization and death, including in South Africa against a more transmissible variant.
5) “When moderate cases were included, however, it was 66 percent protective, compared to efficacy of more than 90 percent reported for a vaccine jointly developed by U.S. pharmaceutical giant Pfizer and German biotech firm BioNTech and one from U.S. biotech company Moderna...
6) “Trials were conducted at different points during the pandemic, and in different countries with different transmission rates, which makes head-to-head comparisons impossible.”
7) “The apparent differences, Baker said, could nonetheless create uncomfortable questions for state and local leaders promoting the new vaccine to people who might ask, as one person paraphrased his comments, “Why didn’t you give us the good stuff?”
8) ““J&J is going to be a challenge for all of us,” WA Gov Jay Inslee.
In N Dakota, which has achieved one of the fastest rates of inoculation, Gov. Burgum said the new product intensified concerns not just about “vaccine hesitancy, but the potential for brand hesitancy as well.”
9) “The problem has been on display in Germany, where some residents are shunning shots developed by AstraZeneca in favor of those made by Pfizer-BioNTech, because of the different levels of protection reported in clinical trials, according to officials there.
10) “If we end up with a hierarchy that says all rich White people get Pfizer, and all poor Black people get J&J, that would be a problem,” said Helene D. Gayle, president and chief executive of the Chicago Community Trust
11) Also let’s look at the direct data given to the FDA... For severe outcomes it was 85% with 95% CI upper bound up to 96.9%. Hence it overlaps with the higher values for Pfizer and Moderna. This lower value seen for J&J could still be roughly comparable!

12) I literally design trials as one of the things I do as an epidemiologist, just like @VincentRK. If these are the results in a regular trials 85% vs 94-97%, I would not say J&J is inferior. (Besides, non-inferiority trials take a long time to do to truly prove not inferior)

⚠️BRAZIL COVID EMERGENCY—Entire 🇧🇷 is now “Manaus”. 1700+ deaths in 24 hours, ICUs critical.

📌#P1 🇧🇷 variant is ~2x-2.5x more transmissible, says @obscovid19br

📌17 state capitals ICUs >80% full

📌”Exponential #COVID19 growth unlike any other that we have seen” @bollemdb 🧵 Image
2) Brazil is becoming an enormous Manaus" - @MiguelNicolelis
3) One group puts #P1 at an astoundingly high 2x more transmissibility.

Experiences from Manaus - the Amazonian city hit hard by the P.1 variant - suggest it could be up to twice as transmissible as earlier Covid there, the first detailed study suggests.

If kids under age 6 can wear masks 102 years ago in the last great pandemic, then kids today can too.

PASS IT ON. #COVID19 #MaskUp #schoolmasks Image
2) We now have major outbreaks in kids worldwide from #B117. See thread 🧵
3) This is the best example of what happens when kids don’t mask in schools....

KIDS EPIDEMIC in Italy 🇮🇹—#B117 variant among Italy's infected schoolchildren and is helping to fuel a “robust” uptick in the curve of #COVID19–with higher transmission rates "among the youngest age group” of the population.

2) In recent weeks, Italy’s incidence of new cases among young people has now eclipsed incidence among the older population, a reversal of how COVID-19 afflicted residents in the first months of the pandemic.
3) Speranza announced stricter directives, contained in the first anti-pandemic decree of new Italian Premier Mario Draghi, aimed at trying to “govern this curve of contagion,” especially among school-age children.

🇸🇪EXPERT HID SCHOOL OUTBREAKS & EXCESS DEATHS IN KIDS, emails reveal—A Swedish proponent of pro-natural-infection-herd who advised Tegnell was caught deliberately omitting data on school outbreaks & excess deaths, as new whistleblower emails shed light.🧵

2) “Ludvigsson was one of 47 original signers of Great Barrington Declaration, a controversial document published in Oct 2020 that argued that pandemic policies should focus on protecting the vulnerable while rest of the population builds up immunity through natural infection.”
3) “Ludvigsson’s research seemed to support those ideas. In a review about children’s role in the pandemic, published in Acta Paediatrica in May 2020, he reported there had been “no major school outbreaks in Sweden,” which he attributed to “personal communication” from Tegnell.

📍Bad warnings from South Africa’s CDC—#B1351 spike protein completely escapes 3 classes of therapeutic antibodies. It also exhibits “substantial to complete escape” from neutralization by convalescent plasma. It bodes Reinfection & lower vaccine efficacy.

2) The @nicd_sa authors conclude: “These data also have implications for the effectiveness of SARS-CoV-2 vaccines, largely based on immune responses to original spike protein.”
3) “Indeed, sera from the Moderna and Pfizer-BioNTech vaccinees show significantly reduced neutralization of 501Y.V2” (#B1351)

WORRIED—NYC’s native variant is now concerning Fauci—it has some potential for trouble. 735 cases of #B1526 variant that emerged in New York City have now been identified in US, including 585 in the last 2 weeks—it is now found in 15 states.🧵#COVID19 bloomberg.com/news/articles/…
2)“It’s not particularly happy news,” said Michel Nussenzweig, an immunologist. “But just knowing about it is good because then we can perhaps do something about it.”
Nussenzweig said he was more worried about variant in New York than one in California.

3) The variant, known scientifically as B.1.526, likely started off in the Washington Heights section of Manhattan, Anthony Fauci said in a Monday news briefing. It is one of five concerning variants now being tracked nationally by health officials.
