.@BradRovin speaking on the evolution of #lupus therapy over the years in @isn_india Masterclass
The main change which has happened is to decrease steroids.
Probably 1 gm of MPS is enough to begin treatment in #lupus
Discussing the data on minimizing steroids @BradRovin shows data by @kidneydoc101 which formed the basis of RITUXILUP
Are we going towards NO steroids for #lupus ?
Hope at the end of the tunnel. Positive trials upcoming new Rx for #lupus
Is obinutuzumab better than rituximab
How is #Voclosporin different from CSA?
Less need for TDM monitoring
high potency
low glucose / lipid side effects
.@BradRovin discusses pooled AURA - LV and AURORA trial
Very hopeful results
Check out #visualabstract and summary in @LastNeph Also, soon to be discussed in @NephJC
Subgroup analysis of AURORA
Is it same for all races and all LN Class? pts on MMF had more significant different (can be related to disease severity)
#voclosporin might not be the first choice for pts with severely reduced GFR
Moving to Belimumab - It reduces flares (even in kids) and slows progression to ESKD when added to background therapy
.@BradRovin is very hopeful about Obintuzumab and 2 doses of MPS with MMF might make #lupus pts steroids free!! @kidneydoc101
He is also talking about subtyping pts who have B cell dominant disease to target for these therapies

How to cope when the whole family is down with #COVID19
Lessons I learned the hard way with 5 family members positive (including my baby) 🧵1/
1/It's very infectious - Try to isolate ASAP when the 1st member gets symptoms.
A separate toilet is mandatory. Keep utensils separate.
Try to shift the elderly to another place. Our parents got infected after D 5 as we were in overcrowded Mumbai house.
2/ Kids can get very fatigued - ensure hydration and nutrition - putting an IV line for this made a big difference (only when she was not feeding at all). Also, because I was too sick myself (despite trying my best I couldn't get her to eat)

Q-Do kidney diseases affect women differently?
2/17
Women have
⬆️Autoimmune diseases (SLE, RA, Systemic Scleroderma)
In #Women of Color that's 2- 3X commoner, early-onset with⬆️ complications

⬆️UTI due to short urethra
In #Pregnancy ⬆️risk of pyelonephritis and sepsis

3/17 Pregnancy poses a high risk of #kidney injury due to various etiologies (see pic) which can progress to CKD in the future.
#Preeclampsia is a definite risk factor for #CKD and #ESKD in mother and also for #cardiovascular disease
