1/ An article from @TheEconomist claiming that India's true #covid19 mortality maybe 5-7 times than the reported numbers has really irked GoI so much so @MoHFW_INDIA has now published a rebuttal that is short on substance but abundant on rhetoric.
2/ After using words like "speculative", "misinformed", "unsound" etc. to describe the article, the rebuttal interestingly says "there is no peer-reviewed scientific data available" for some studies used in that report.

I'm glad GoI takes scientific peer review seriously.
3/ Then why has it made claims on the safety & effectiveness of Covaxin without backing up with peer-reviewed scientific data?

What was the peer-reviewed scientific data based on which #Coronil got Ayush Ministry certification?

Thi list is actually long. So, I'll stop.
4/ The rebuttal says GoI "has been transparent in its approach to COVID data management" & MoH has
"robust reporting mechanism" for "district-wise cases & deaths on a daily basis".

Where do you report daily district-wise data on #COVID19 cases & deaths for public access?
5/ Rebuttal says ICMR has "issued ‘Guidance for appropriate recording of COVID19 related deaths in India’ for correct recording of all deaths"

Do you have data to show which states adhere to these guidelines & which do not? What actions have you taken against those who didn't?
6/ There is sufficient data in the public domain that most states in India do not follow @ICMRDELHI guidelines on reporting of COVID19 deaths & yet, there haven't been any decisive actions taken so far to correct those.
7/ The rebuttal interestingly points to GoI's writing to the state of #Bihar to provide a detailed date & district-wise break-up of the reconciled number of COVID19 deaths as an example of its robust reporting mechanism for monitoring cases and deaths.
8/ However, it conveniently omits the fact that Bihar was forced to do this reconciliation in reported covid deaths after a Patna High Court last month that flagged irregularities in reporting of covid deaths in the state.
9/ Interestingly the rebuttal acknowledges there will always be a difference in mortality recorded during a pandemic vs the actual mortality.

Has GoI made any official estimate of the true covid mortality from its first wave since we've been already many months past it?
10/ Many epidemiological models have estimated that #COVID19 deaths in India to be ​~5 times or more than the reported. So, the estimate in @TheEconomist report is well within what the scientific community already believes to be the true extent of the excess deaths in India.
11/ There have also been other excess mortality estimates in select states/regions (using official all-cause mortality data) which puts the actual death tolls to be 5 times high. Here is the latest example of that from Madhya Pradesh as done by @Rukmini.
12/ Instead of issuing rebuttals of this kind (pib.gov.in/PressReleasePa…), @MoHFW_INDIA must take proactive measures to facilitate timely estimation of excess deaths during pandemics so that we get a reasonably accurate picture of the true extent of deaths & devastation.

Estimation of such data is already possible in several countries as done here. But, huge delays & the absence of regular monthly data on all-cause mortality across most Indian states limit India's ability to get up-to-date & accurate data on deaths.

14/ There is no point blaming anyone trying to make sense of the reality of the true devastation of this pandemic with studies that are often limited by the availability of good data and not by the rigor of methods used.
15/ Instead, the GoI must proactively & regularly publish reliable all-cause mortality data in regular intervals at the national level so that studies using incomplete & unreliable data to make sense of the true extent of deaths in a pandemic become redundant.

Data thread on #COVID19Vaccine distribution across Indian states

*Age group:
HCWs & FLWs are among all age groups

Most of the limited supply should ideally go to the 60+ followed by 45-60 until supply is sufficient. Yet, a large chunk is going to 18-45 in some states

1/
There is a considerable gender gap in the distribution of vaccinations in some states

2/
88% of all doses so far were Covishield at the national level, followed by Covaxin. Sputink already distributed in some states such AP, TS, MH, KA, KL, & WB.

1) #Kerala's #COVID19 mortality rate has been consistently increasing over the past month after reaching a low of 0.3%. Still one of the lowest in India.

Total reported #COVID19 deaths are at 10,157, the 10th largest in India.
2) There are concerns of underreporting of deaths with mortality rates being significantly different across districts ranging from 0.1% to 0.8%.

During the 2nd wave from March 18 in KL, the share of total cases from Trivandrum is only 10% while that of deaths is 22%.

1/ This was the thing that greeted me on my WhatsApp today morning. Let's examine its utter folly.

Is India's Vaccine Maitri the reason we are getting all those foreign aid? I do not think so.

why?
2/ First of all, of the total vaccine exports from India till y’day, only ~16% were sent as Grants by GoI. Others were all SII fulfilling its commitments one way or the other. GoI has nothing to do there or take credit for.
If anything, GoI had to stop SII from exporting further.
3/ Even out of this 16%, about 65% went to 5 of our neighbors (Bangladesh, Myanmar, Nepal, Bhutan, & Maldives). The remaining went mostly to other low-income coutries.

@MoHFW_INDIA is the official Twitter handle of the Ministry of Health & Family Welfare, GoI. It must a place to report accurate, realistic, & contextual info on the pandemic that is ravaging the country!

Has it done that? I do not think so!

Why?

There's been an undue emphasis on the cumulative numbers of #COVID19 vaccine doses administered, tests done, recovered cases, etc. without a sense of reality. India is the 2nd populous country in the world. So, the cumulative numbers for anything ought to be large!

Instead, @MoHFW_INDIA should instead answer:

What % of our population has been vaccinated so far?

What has been the pace of vaccination?

By when do you think the vaccination coverage can reach the target?

When do you think an ordinary citizen would be able to get a jab?

Thread on COVID testing data

It's been a month since @ICMRDELHI has been reporting at least 100K fewer tests than the tests reported by all states combined. This discrepancy has been growing & is over 3L for the past several weeks.

What does it mean? What implications?

In the past, this difference used to get reconciled over time. But, this time, this difference is so large & not been reconciled for a long time.

Few plausible reasons:
1) ICMR only reports tests coming from its own approved labs & states maybe adding testing numbers from unrecognized labs too
2)States inflating the testing numbers on paper for various reasons

Thread on #COVID19Vaccination

Daily avg jabs are now 1.8M indicating acute vaccine shortage whether GoI admits it or not.

It means 18-45 yrs who are better positioned to register on CoWIN & at lesser risk of dying are using the limited doses & depriving the more vulnerable

Avg. daily jabs have declined from a peak of 3.7m to 1.8m (51% fall over the past month)

Many vulnerable & aged have to reschedule their visits multiple times to get jabs which increase their risk of exposure significantly

Until the vaccine supply is increased & can be ensured at a minimum of 5m doses/day, the GoI should immediately stop the 18+ vaccination drive.

This is extremely important to reduce the risk of deaths among the most vulnerable population.
