They don't actually say what "ethnicity" the woman and the child are that Cindy McCain saw. But I'm going to take a guess that the woman was darker than the child — and that McCain only has a problem with that, not the other way around.
It's important to note that racism doesn't require you to hate dark-skinned people — only to think you're better than them.

That's why you could be friends with, or even marry or adopt, people of color, and still be a racist.
If you don't hate POC, but do think that they are in general inferior to white people, you'd find it completely fine — virtuous, even — for a white family to adopt a dark-skinned child because in that logic that child is being elevated.

But god forbid it be the other way around.
