We invested in pop health for 3 years before we got a check from Medicare.
3/ The Aledade Mississippi ACO, with practices in throughout the state and southwest Tennessee, reduced inpatient hospitalizations by 9 percent, saved Medicare $5.2 million, and earned a quality score of 95 percent

Blood pressure control rates are up to 79% in Mississippi Delta!
4/ The Aledade Florida Central ACO, centered in the greater Orlando area, reduced inpatient hospitalizations by 8 percent, saved Medicare $3.4 million, and earned a quality score of 96 percent.

6/ The Aledade Pennsylvania PMA ACO reduced emergency department visits by 17 percent and inpatient hospitalizations by 21 percent, saved Medicare $15.5 million, and earned a quality score of 99 percent.

We started with a healthy population, low hospitalization and readmission rate. Doesn't matter.

If you want to see "social drivers" talk to these FQHCs. They prove that value based care is possible for all populations.
10/ But what's almost more important is that there is not a single @AledadeACO that did not make meaningful and measurable improvements in patient outcomes.

Appalachia ACO reduced ED visits by 15%, Gateway ACO by 14%
