"We concluded that the failures described above and in this report represent serious performance failures by the supervisory and non-supervisory agents with responsibility over the FISA applications."

Damning statement.
2) "These failures prevented OI from fully performing its gatekeeper function and deprived the decision makers the opportunity to make fully informed decisions."
3) "We believe that all of them ["factual misstatements and omissions"] taken together resulted in FISA applications that made it appear that the information supporting probable cause was stronger than was actually the case."

1) On OGC Attorney email falsification - presumably Clinesmith:

"In an email from the liaison to the OGC Attorney, the liaison provided written guidance, including that it was the liaison's recollection that Page had or continued to have a relationship with the other agency."
2) The liaison "directed the OGC Attorney to review the information that the other agency had provided to the FBI in August 2016. As noted above, that August 2016 information stated that Page did, in fact, have a prior relationship with that other agency."
3) "following a 28 minute telephone call between the OGC Attorney [Clinesmith] and the OI Attorney, the OGC Attorney forwarded to the OI Attorney the liaison's email (but not the original email from the OGC Attorney to the liaison setting out the questions he was asking)"

The news is out that @Comey did indeed have some credible prosecutors for Hillary Clinton's mishandling of classified documents during her stint as secretary of state.
